Entrevista/ Dr. Kenji Ojima, presidente de la Academia Japonesa de Ortodoncia con Alineadores (JAAO)

El Dr. Kenji Ojima, todo un referente a nivel mundial en el campo de la ortodoncia, nos cuenta en esta entrevista los últimos avances en el sector de ortodoncia con alineadores y las oportunidades que presentan para las maloclusiones de pacientes en crecimiento, incluyendo las coronas clínicas cortas, el control de la erupción dental y la expansión de la arcada.

DM.- Align Technology ha sacado al mer-cado el tratamiento Invisalign First, diseñado para pacientes con dentición mixta temprana. ¿Qué otras ventajas cree que aporta Invisalign First, un producto especialmente diseñado pa-ra pacientes en crecimiento?Dr. Kenji Ojima.- Aunque Invisalign First se ha desarrollado especialmente para pacientes en la primera fase, incluye todas las ventajas que tienen los aligners trasparentes Invisalign tradiciona-les. Es predecible y está adaptado a cada paciente, de manera que desde el principio el ortodoncista puede anticipar cómo evolucionará la dentadura del paciente. DM.- Align Technology has recently relea-sed Invisalign First treatment, designed for the patients with early mixed dentition. What other advantages do you think Invisalign First has, besides a product especially designed for patients who are still in their growing phase?Dr. Kenji Ojima.- Of course, while Invisalign First has been developed especially for patients in Pha-se 1, it includes all the advantages that the tradi-tional Invisalign clear aligners have. It is highly accurate and customized for each patient, so from the very beginning, the doctor can anticipate how the patient dentition will evolve. In addition, Invi-salign First includes high-tech features developed by Align Technology to address a broad range of younger patients’ malocclusions, including shor-ter clinical crowns, management of erupting den-tition, and predictable dental arch expansion.

DM.- Últimamente hay muchas casas co-merciales que anuncian alineadores hechos a partir de registros digitales o convencionales sin relación directa con el profesional de la ortodoncia. ¿Qué opinión le merece esta nue-va tendencia? Dr. K. O.- En primer lugar, es importante recordar que con los aligners trasparentes no solo trabaja-mos para lograr una sonrisa bonita, que es muy importante, sino que mejoramos y cuidamos de la salud bucodental. pacientes, antes de aventurarse a cualquier trata-miento con aligners, hablen con un dentista u or-todoncista especializado, que nunca les recomen-dará un tratamiento adquirido online que no esté supervisado por un profesional de la salud debi-damente certificado, dado que dichos tratamien-tos pueden impactar negativamente en la salud. Los dentistas y ortodoncistas somos expertos en nuestras áreas de especialización y estamos cua-lificados para hacer recomendaciones médicas y prescripciones a nuestros pacientes sobre cuál es la mejor solución para sus problemas dentales. DM.- During last months, there have been companies advertising aligners made from di-gital registers or conventional material wi-thout the involvement of an orthodontic pro-fessional. What is your opinion on this new trend? Dr. K.O.- First of all, it is important to remember that with clear aligners we are not only working with achieving a beautiful smile, which is very important, but also with oral health. This is why it is important for patients, before embarking on any aligner treatment, to speak to a trained den-tist or orthodontist, who will not recommend a treatment purchased online that is not supervised by a fully certified healthcare professional, since such treatments could have negative effects. Den-tists and orthodontists are experts in their field - we are qualified to make medical recommenda-tions and prescriptions to our patients as to what is best solution for their specific and unique den-tal problems. Clear aligner treatments produced by Align Technology are manufactured based on a doctor’s prescription and customized for each pa-tient, straightening teeth with high precision and accuracy.

In fact, using an orthodontic appliance that has not been prescribed by a dentist or orthodontist may not work, or in any case it may not work as well as a treatment supervised by a professional. We are talking not only about moving teeth but al-so about the retention time, which means that the orthodontic devices have to be perfectly designed to modify a patient’s bite exactly as they need wi-thin the right timeframe.

DM.-Sin embargo, algunos de estos productos dicen que pueden alinear los dientes de un pa-ciente en una media de 20 semanas. Al final, se pueden considerar todos aligners también, ¿no? Dr. K. O.- El tiempo de tratamiento depende de muchos factores -como de grave es la maloclusión, el tipo de tratamiento (ej. estético vs funcional), el cumplimiento por parte del paciente, etc. Por ejemplo, los aligners transparentes de Align Tech-nology permiten al dentista decidir si el paciente debe cambiar los aligners de forma quincenal o se-manal. En el caso del cambio de aligner semanal, el tiempo de tratamiento puede reducirse incluso un 50 por ciento -dependiendo del caso- lo que sig-nifica que con algunos tratamientos, los primeros resultados se pueden ver después de tan solo tres meses. La supervisión profesional es clave, pues siempre es el Doctor quien decide cuál es el tra-tamiento adecuado, y cuál es el tiempo de trata-miento apropiado.

DM.-However, some of these products claim to straighten patient’s teeth in an average of 20 weeks. At the end of the day, we are talking about clear aligners as well, right?Dr. K.O.- The treatment time depends on many factors – how severe the malocclusion is, type of treatment (eg. aesthetic vs. functional), patient̀ s compliance, etc. For example, Align Technology`s clear aligners allow dental professional to decide if their patient should change the aligners on a bi-weekly or weekly basis. In case of weekly alig-ner change, the time of the treatment can be redu-ced even by 50 percent – depending on the case – which means that with some types of treatments, first results can be seen after as little as three months*. Professional supervision is key, as it’s always the doctor who decides what is the right treatment, and what is the right treatment time.

DM.-.- Respecto a las impresiones digitales, ¿considera que los escáneres intraorales como iTero Element son imprescindibles para todos los casos o aún queda espacio para la silicona y el alginato de toda la vida? Dr. K. O.- Los aligners trasparentes están siguien-do un proceso de evolución parecido a otras inno-vaciones de la historia. Por ejemplo, hace 30 años

los teléfonos tenían cables y variaban muy poco en cuanto al diseño, y normalmente se encontraban en casa. Sin embargo, hoy llevamos nuestro propio teléfono en el bolsillo y, además, podemos navegar por internet, jugar, escribir emails, etc. Igual que con el teléfono, la digitalización y la tec-nología han facilitado la vida tanto de los ortodon-cistas como de los dentistas, además de los pacien-tes, y el futuro será definitivamente digital. Ya se han visto ejemplos de la integración de sistemas de alta tecnología en el campo de la ortodoncia.Con el escáner iTero Element, los profesionales pueden generar un escaneado a color que muestre la dentición para una precisa evaluación clínica de los pacientes, facilitando la comunicación con el paciente y, por lo tanto, generando una mejor com-prensión del plan del tratamiento, lo cual sabemos que aumenta el número de pacientes que lo eligen. DM.- Regarding digital impressions, do you think that intraoral scanners, such as the iTe-ro Element, are essential to all cases or is the-re still room for the classical silicone and al-ginate treatments? Dr. K.O.- Clear aligners are following similar rou-te as other innovations in history. For instance, 30 years ago telephones had cables and varied very little in terms of design, and they were usually found at home. However, nowadays, we carry our own phone in our pocket and, in addition, we can navigate the web, play games, write emails, etc. As with the phone, digitalization and technology have made lives easier for both dentists and ortho-dontists as well as patients and the future will de-finitely be digital. We have already seen examples of the integration of high technology systems in the orthodontic field.With iTero Element scanner, professionals are able to generate a coloured scan that visualizes the dentition for a precise clinical evaluation of

patients and facilitating patient communication, which we know can drive case acceptance becau-se of better understanding of the treatment plan. This kind of scanners provide a dental “chair-si-de” platform facilitating access to digital diagnosis and treatment tools, with potential for enhancing the overall patient experience much further than the classical methods; whether it’s a crown or bri-dge restoration, full or partial orthodontics (inclu-ding clear alignment treatments), veneers or a va-lidated implant solution, practitioners can select the workflow option that best fits their needs for each individual patient case. DM.-.- Align Technology ha revolucionado el campo de la ortodoncia, y el sector dental en general, ¿podríamos afirmar que estamos ante un salto evolutivo en el campo de la or-todoncia? ¿Cuáles son las ventajas de este sis-tema? Dr. K. O.- El sistema Invisalign es el primer siste-ma de aligners trasparentes que ha revoluciona-do el mundo del sector ortodóntico y ha demostra-do definitivamente un avance enorme, por varios motivos. En primer lugar, como los aligners son removibles, los pacientes pueden mantener una mejor higiene bucodental ya que la limpieza no queda inhibida como tampoco otras actividades cotidianas como comer o practicar deporte, ya que el aligner se puede retirar en cualquier mo-mento. Además, como son trasparentes, el pacien-te se siente más cómodo llevándolos que con los brackets de metal, ya que los aligners no suponen un cambio en su aspecto. Pero hay muchas otras ventajas a un nivel más técnico. Por ejemplo, el escaneado 3D y las técnicas de impresión digital que se utilizan para diseñar y fabricar los alig-ners del Sistema Invisalign permiten a los dentis-tas ofrecer un tratamiento mucho más personali-zado, adaptado a las necesidades particulares de cada paciente y garantizar un producto de alta ca-lidad. Además, los esfuerzos en I+D han hecho posible que algunos aligners trasparentes abar-quen una amplia gama de maloclusiones que an-tes no podían tratarse con este sistema, especial-mente en el caso de Invisalign Comprehensive, que puede utilizarse para tratar maloclusiones le-ves y graves.

DM.- Align Technology has revolutionised the orthodontic sector, and the dental sector in general. Could we say that we are facing an evolution in orthodontics? What are the ad-vantages of this system? Dr. K.O.- The Invisalign system was the first clear aligner system to shake up the world of orthodon-tic sector and has definitely proven a huge step forward, for a number of reasons. First, as the alig-ners are removable, patients can maintain better oral hygiene as cleaning is not inhibited, and nei-ther are other daily life activities such as eating or playing sports, as the aligner can be removed at any time. What’s more, as they are transparent, patients are more at ease wearing them than for example metal braces, as the aligners do not chan-ge their outward appearance. But there are also many other advantages on a more technical level. For instance, 3D scanning and digital printing te-chniques used to design and produce Invisalign aligners allow dentists to offer a much more perso-nalized treatment, adapted to each patient’s parti-

cular needs and guaranteeing a high-qua-lity product. In addition, the R&D efforts have made it possible for some clear alig-ners to cover a wide range malocclusions that were not previously possible, especia-lly in the case of Invisalign Comprehensi-ve treatment, which can be used to treat more complex types of malocclusions.

DM.- Si le tuviera que pedir una in-novación a la empresa Align Technolo-gy, ¿cuál sería?Dr. K. O.- Más que una innovación, yo le pediría a Align Technology que continúe desarrollando una línea que ha empezado a explorar hace poco y que hemos mencio-nado al principio de la entrevista: los tra-tamientos para los pacientes en crecimien-to. El aligner trasparente Invisalign First ha sido un gran paso adelante, ya que los aligners trasparentes no estaban diseña-dos para los pacientes que no habían per-dido todos sus dientes primarios. Vemos cada vez más pacientes en crecimiento que acuden a nuestras clínicas, ya que tanto ellos como sus padres son conscientes de la importancia de enderezar los dientes a una edad temprana para beneficiarse en la edad adulta. Por eso, creo que es funda-

mental continuar desarrollando tratamientos para pacientes en crecimiento, para que podamos ofre-cerles un futuro mucho más sano.

DM.- If you would need to ask Align Tech-nology for one innovation, which one would it be? Dr. K.O.- More than an innovation, I would ask Align Technology to continue developing the li-ne that has started exploring recently and that we mentioned at the beginning of the interview: treat-ments for younger patients. The Invisalign First clear aligner has been a huge step forward, as clear aligners were not designed for patients that had not lost all their primary teeth. We are seeing more and more young patients coming to our cli-nics as they and their parents are aware of the im-portance of straightening the teeth at an early age to generate benefits in their adult lives. For this re-ason, I think is vital to continue developing treat-ments for young patients, so we can offer them a much healthier future.
