RESUMO Aloe ferox Miller, pertencente à família Liliaceae, é originária da Província do Cabo Oriental na África do Sul. No Brasil, a planta é conhecida como babosa e os maiores produtores encontram-se no interior de São Paulo (particularmente no município de Jarinu), Santa Catarina e na região Nordeste. Sendo esta última, a que possui as melhores condições de plantio. Ela consiste em um arbusto perene arborescente típico de climas secos e quentes, possui folhas alongadas e pontiagudas, e é constituída em duas partes: gel e látex. A resina de Aloe ferox é um resíduo sólido obtido pela evaporação do látex que escorre do corte transversal de suas folhas. Na medicina popular, dentre outras aplicações, a resina da planta é usada no tratamento da constipação. Os efeitos da administração oral de A. ferox foram investigados sobre a fertilidade, prenhez e desenvolvimento pós-natal da prole de ratas Wistar. A aloína presente na resina foi identificada por cromatografia em camada delgada e os derivados hidroxiantracênicos expressos como aloína foram quantificados por espectrofotometria. A resina na forma de pó foi dissolvida em glicerina 40% (v/v). O estudo foi realizado em três períodos, cada um contendo cinco grupos de ratas prenhes (n=8-9/grupo), totalizando 15 grupos randomicamente formados. Os grupos 1 e 2 (grupos controle) receberam água destilada e solução de glicerina 40% (v/v), respectivamente, enquanto os outros foram tratados oralmente com A. ferox nas doses de 0,1, 0,5 e 2,5 g/kg. No primeiro período, o tratamento foi administrado do 1º ao 6º dia (período de pré-implantação - PP) e o segundo, do 7º ao 14º dia (período de organogênese - PO). No 20º dia de prenhez, as ratas foram laparotomizadas para avaliação dos parâmetros reprodutivos. No último período, as ratas prenhes foram tratadas oralmente com as mesmas doses durante toda a prenhez e os parâmetros maternos e os da prole foram avaliados. A aloína (Rf 0,35) foi identificada e a porcentagem dos derivados hidroxiantracênicos expressos como aloína foi 33.5%. Durante o PP houve diminuição no ganho de massa corporal materna (p < 0,05) em todas as doses. Reduções também foram observadas em alguns parâmetros maternos (massas da placenta) e fetais (comprimento e massa relativa) nas doses de 0,5 e 2,5 g/kg quando comparado aos grupos controle. No PO, a redução no ganho de massa corporal materna foi observada apenas no tratamento com a maior dose. Entretanto, outros parâmetros como massas dos fetos, da placenta e dos ovários foram alterados em todas as doses avaliadas. Durante o período integral da gestação houve aumento apenas na massa corporal e comprimento dos conceptos no 7º e 21º dias de vida pós-natal, na maior dose. Os parâmetros comportamentais da prole não foram alterados. Dessa forma conclui-se que o uso da glicerina como solvente não interferiu nos parâmetros reprodutivos analisados no estudo. Embora alguns parâmetros tenham sido alterados pelo tratamento com A. ferox, o desenvolvimento normal da prole não foi influenciado por ele. Mas, o tratamento com a maior dose de A. ferox sugere possível toxicidade materna devido ao provável efeito abortivo obtido com essa dose, mesmo sem causar morte de ratas prenhes e malformações fetais. Nesse sentido, estudos posteriores devem ser conduzidos a fim de assegurar o uso dessa planta durante a gestação humana. Palavras-chave: Aloe ferox Miller, Aloína, Toxicologia; Prenhez, Prole.

ABSTRACT

Aloe ferox Miller (Liliaceae) is widespread in the Eastern Cape Province of South Africa. In Brazil, the plant is popularly known as babosa and the largest producers are in São Paulo (mainly Jarinu city), Santa Catarina and Northeast region. This region has the best conditions for planting. It is arborescent perennial shrub that grows in hot and dry climates, it is made of elongated and pointed leaves and it consists of two parts: gel and latex. Aloe ferox resin is the solid residue obtained by evaporating latex which drains from the transversally cut leaves. In folk medicine, among other applications, the resin of the plant is used in the treatment of constipation. The effects of the oral administration of A. ferox were investigated on fertility, pregnancy and offspring postnatal development of female Wistar rats. The aloin was identified by thin layer chromatography and hydroxyanthracene derivates expressed as aloin were quantified by spectrophotometry. The resin in powder form was dissolved in glycerin 40% (v/v). The study was conducted in three periods, each one containing five groups of pregnant rats (n = 8-9/group), in total of 15 groups of rats randomly formed. The groups 1 and 2 (control groups) received distilled water and glycerin solution 40% (v/v), respectively, while the others were treated orally with A. ferox at doses 0.1, 0.5 and 2.5 g/kg. In the first period, the treatment was administered from 1st to 6th day (preimplantation period- PP) and the second, from 7th to 14th day (organogenesis period - OP). On day 20th pregnancy, the rats were laparotomized for evaluation of maternal and fetal parameters. In the last phase, the pregnant rats were treated orally with the same doses during whole pregnancy and maternal and offspring parameters were evaluated. The aloin (Rf 0.35) was identified and the percentage of hydroxyanthracene derivates expressed as aloin was 33.5%. During the PP there was reduction in the maternal body mass gain (p < 0.05). Reductions were also observed in some maternal (mass of the placentae) and fetal (length and relative mass) parameters at doses 0.5 and 2.5 g/kg when compared with control groups. In the OP, the reduction in the maternal body mass gain was observed only at the highest dose. However, other parameters like masses of fetuses, placentae and ovary were changed at all doses evaluated. The maternal reproductive parameters during whole pregnancy showed increases only in the pups body mass and length on days 7th and 21th postnatal life, respectively, at the highest dose. The offspring behavioral parameters were not changed. Thus, it concludes that the use glycerin as solvent did not interfere in the reproductive parameters analyzed in study. Although some parameters have been changed by treatment with A. ferox, the offspring normal development was not influenced by it. Nevertheless, the treatment with A. ferox at the highest suggests some maternal toxicity due to possible abortive effect shown in this dose, even without cause deaths of pregnant rats and fetal malformations. 1.0. Introdução

A utilização de plantas com fins medicinais para o tratamento, a cura e a prevenção de

doenças é uma das mais antigas formas de prática medicinal da humanidade (PINTO, 2002).

A terapêutica moderna, composta por medicamentos com ações seletivas sobre

receptores, enzimas e canais iônicos não seria possível sem a contribuição dos produtos

naturais, notadamente das plantas superiores. Essa informação pode ser facilmente

comprovada quando se analisa o número de substâncias obtidas direta ou indiretamente a

partir de fontes naturais. Atualmente, os produtos naturais são responsáveis por cerca de 40%

dos fármacos disponíveis para o tratamento de várias doenças, além do que há um grande

interesse das indústrias farmacêuticas pelo uso da biodiversidade como fonte de novos

fármacos (CALIXTO, 2003). Assim sendo, a fitoterapia constitui uma forma de terapia

medicinal que vem crescendo nos últimos anos e que movimenta no mercado mundial U$$ 22

bilhões anuais (PINTO, 2002). Em 2006, as vinte maiores empresas de fitoterápicos do Brasil

venderam aproximadamente R$ 460 milhões, valor equivalente a 84,7% do total faturado pelo

segmento (BRASIL, 2007).

Segundo a Organização Mundial de Saúde (OMS), 80% da população mundial faz uso

de terapias alternativas como formas de tratamento e as plantas medicinais são os principais

medicamentos utilizados. Apesar do uso tradicional e do elevado consumo de plantas

medicinais, apenas isso não comprova a sua efetividade nem a ausência de toxicidade, é

necessário conhecer a dose eficaz e aquela que produz toxicidade, portanto, é fundamental o

estudo farmacodinâmico e toxicológico padronizado (AGRA et al., 2007).

A toxicidade de plantas medicinais é um problema sério de saúde pública. Os efeitos

adversos dos fitomedicamentos, as possíveis adulterações e toxicidade, bem como a ação

sinérgica com outras drogas ocorrem comumente. As pesquisas realizadas para avaliação do

uso seguro de plantas medicinais e fitoterápicos no Brasil ainda são incipientes, assim como o

controle da sua comercialização pelos órgãos oficiais (VEIGA-JÚNIOR et al., 2005).

O gênero Aloe é amplamente empregado na medicina tradicional para o tratamento de

diversas doenças, entretanto em revisão de literatura não se detectou a presença de estudos

sobre segurança de uso, particularmente durante o processo reprodutivo da espécie Aloe ferox.

Desta forma, esse estudo procura explorar o efeito da resina dessa espécie durante o período

de gestação em ratas Wistar e o desenvolvimento dos conceptos.

2.0. Revisão Bibliográfica

2.1. Aloe ferox Miller

2.1.1. Aspectos botânicos

Aloe ferox Miller, pertencente à família Liliaceae, é originária da Província do Cabo

Oriental na África do Sul (ZAHN et al., 2007). No Brasil, a planta é popularmente conhecida

como babosa e os seus maiores produtores encontram-se no interior de São Paulo

(particularmente no município de Jarinu), Santa Catarina e na região Nordeste. Sendo esta

última, a que possui as melhores condições de plantio (MAGALHÃES, 2005). Consiste em

um arbusto perene arborescente típico de climas secos e quentes. Possui um único tronco que

pode atingir 2-3 m de altura, sendo caracterizado por possuir folhas secas persistentes nas

porções mais baixas do caule. As folhas largas são verdes no verão ou verde-avermelhadas no

inverno, com espinhos marrom-escuros ao longo das bordas. Apresenta inflorescência

racemosa ereta medindo cerca de 60 cm de altura, com flores que podem ser vermelhas,

alaranjadas ou amarelas como mostrado na figura 1 (PALGRAVE, 1984).

A planta consiste de dois produtos básicos: gel e látex, como mostrado na Figura 2

(BERTI, 2008). O gel de Aloe ferox é a polpa ou a mucilagem da folha, também chamado de

extrato do parênquima de reserva (EPR), consiste de uma substância clara e pouco consistente

obtida da porção interna da folha (TYLER, 1994). O extrato do parênquima clorofiliano

(EPC), também chamado de suco de Aloe, é um látex amarelo de sabor amargo produzido

pelas células excretoras do mesófilo, localizado logo abaixo da epiderme das folhas

(VOGLER; ERNST, 1999).

A resina de Aloe ferox é um resíduo sólido obtido pela evaporação do látex, com ou

sem auxílio do calor, que escorre do corte transversal das folhas (BRUNETON, 1995).

2.1.2. Aspectos fitoquímicos

Aloe ferox é composta principalmente por glicoproteínas, polissacarídeos, substâncias

de baixo peso molecular e antraquinonas. Os polissacarídeos mais comuns são os

glucomanan, que são polímeros de manose, e podem estar acetilados. Os polímeros de

galactose e ácido galacturônico também são frequentemente encontrados. Entre as substâncias

de baixo peso molecular têm-se aloesina, β-sitosterol, dietilhexilftalato, vitaminas e

betacaroteno. As antraquinonas aloe-espcíficas estão também presentes e incluem aloe-

emodina, aloína, barbaloína, isobarbaloína (CHOI; CHUN, 2003). Da raiz da planta foi

isolada a antrona denominada aloe-barbendol (KOCH, 1996).

O extrato do parênquima clorofiliano é formado predominantemente por glicosídeos

de antraquinonas, principalmente aloína. A aloína é um C-glicosídeo da antrona aloe-emodina

e consiste na mistura de dois diastereoisômeros: aloína A (também chamada apenas de aloína)

e aloína B (também chamada de barbaloína) como mostrado na figura 3 (HAYNES, 1960;

TYLER, 1994). A aloína foi caracterizada como 10-β-D-Glucopiranosil-1,8-dihidroxi-3-

hidroximetil-9(10-H)-antracenona e apresenta fórmula molecular C21H22O9 (HAY; HAYNES,

1956; CONNER et al., 1989; YUKO et al., 1990).

A assimetria molecular apresentada pela aloína explica a resistência que ela oferece à

hidrólise, que é uma etapa importante da quantificação dos derivados hidroxiantracênicos

expressos como aloína. Apenas com a utilização de oxidantes como o cloreto férrico,

perborato e peróxido de sódio, por exemplo, consegue-se separar a aloe-emodina-antrona da

parte osídica (arabinose), devido à quebra da cadeia glucosil do C-heterosídeo (COSTA,

1975). Essa análise quantitativa dá-se por espectrofotometria. Já a cromatografia em camada

delgada é empregada na análise qualitativa para verificar a presença de glicosídeos

antracênicos como a aloína (FARMACOPÉIA EUROPÉIA, 1997).

2.1.3. Etnofarmacologia e Estudos farmacológicos

Dados etnofarmacológicos descrevem o uso da resina de Aloe ferox na forma de pó

como estomáquico, emenagogo, anti-helmíntico e laxante (BALBACH, 1986). A resina de

Aloe ferox é obtida na cultura popular deixando-se durante 1 ou 2 dias, as folhas que foram

cortadas pela base, inclinadas umas sobre as outras, com as superfícies seccionadas voltadas

para baixo, sobre dispositivos que as mantenham nessa posição, de modo que o suco escorra

para os recipientes de coleta num nível inferior. Esse sumo é evaporado espontaneamente e

quando bem seco pode ser transformado em pó e dissolvido em água com açúcar, sendo

assim, tomado como laxante (BARRACA, 1999).

O efeito laxativo de Aloe ferox dá-se pela presença de glicosídeos antracênicos

(principalmente aloína), que na flora intestinal são metabolizados à antrona aloe-emodina

(aglicona), que parece agir pelo distúrbio no equilíbrio entre a absorção de água no lúmen

intestinal via um transporte ativo de sódio (ISHII et al., 1990). A aloína inibiu in vitro a

bomba Na+-K+-ATPase presente no cólon dos ratos e aumentou in vivo a permeabilidade

através da mucosa colônica dos mesmos (ISHII et al., 1994). Em experimentos com ratos

verificou-se que laxantes contendo derivados antracênicos podem desenvolver um efeito

carcinogênico na região colo-retal (SUGIE et al., 1994). Entretanto, em ratos F344, o efeito

anti-carcinogênico de Aloe foi observado para esse mesmo tipo de câncer (SHIMPO et al.,

2001; SHIMPO et al., 2006). A atividade antitumoral da aloína isolada e do extrato do

parênquima clorofiliano também foi avaliada in vitro, utilizando células de melanoma de

camundongos. A aloína e o extrato exerceram uma ação tóxica sobre essas células a partir de

concentrações na ordem de 10 µg/mL, alterando a viabilidade celular das mesmas (BERTI et

al., 2008).

A aloe-emodina e a aloína isoladas de Aloe ferox também foram ativas contra as cepas

de Escherichia coli, Staphylococcus aureus, Klebsiella pnemoniae e Pseudomonas

aeruginosa (KAMBIZI et al., 2004). As antraquinonas e seus derivados também são

conhecidos por suas ações anti-inflamatória, imunoestimulante, anti-oxidante e anti-

protozoário (CHOI; CHUNG, 2003). Um provável mecanismo para as ações anti-inflamatória

e imunoestimulante seria a sua ação anti-oxidante. Espécies reativas de oxigênio medeiam a

resposta inflamatória e podem contribuir com a necrose hepática (GRESSNER, 1991). A

análise histológica do fígado de ratos pré-tratados com aloe-emodina mostraram redução da

lesão hepática aguda induzida por tetracloreto de carbono (AROSIO et al., 2000). Desse

modo, a aloe-emodina parece proteger contra a morte do hepatócito e contra a resposta

inflamatória que se inicia logo após a peroxidação lipídica (MALTERUD et al., 1993).

Tem-se observado que as glicoproteínas presentes em Aloe ferox estimulam a

proliferação celular (CHOI et al., 2001). Jia et al. (2008) também demonstraram que os

polissacarídeos presentes nessa planta, principalmente a manose, são capazes de acelerar o

processo de cicatrização de feridas na pele de coelhos e ratos. Isso se daria porque a manose

liga-se a receptores presentes na superfície dos fibroblastos, estimulando-os e promovendo o

rápido crescimento e replicação celular (EAST; ISACKE, 2002). Os polímeros acetilados de

manose também induzem a produção de citocinas que regulam o processo de cicatrização de

feridas (BARBUL, 1990). Em adição, eles também podem exercer efeitos antivirais e

antitumorais in vivo, via ativação de macrófagos, produção de óxido nítrico e ativação dos

linfócitos T citotóxicos (LEE et al., 2001; CHOI; CHUN, 2003).

Em adição a esses compostos, moléculas de baixo peso molecular como a aloesina

inibiu a atividade da tirosinase em humanos, por um mecanismo de inibição não-competitivo,

o que é uma etapa importante na síntese de melanina (JIN et al., 1999). A β-sitosterol mostrou

um efeito angiogênico dose-dependente por melhorar a expressão de proteínas como o fator

de von Willebrand, o fator de crescimento do endotélio vascular (VEGF) e do receptor para

esse VEGF, o Flk-1 (CHOI et al., 2002). O dietilhexilftalato isolado da planta induziu

apoptose celular e demonstrou possuir efeitos anti-leucêmicos e anti-mutagênicos (LEE et al.,

2000).

2.2. Plantas medicinais e toxicidade

A OMS (1998) define planta medicinal como sendo todo e qualquer vegetal que

possui, em um ou mais órgãos, substâncias que podem ser utilizadas com fins terapêuticos ou

que sejam precursoras de fármacos semi-sintéticos (VEIGA-JÚNIOR et al., 2005). No Brasil,

o principal órgão responsável pela regulamentação de plantas medicinais e seus derivados é a

Agência Nacional de Vigilância Sanitária (Anvisa), autarquia do Ministério da Saúde, que

tem como papel proteger e promover a saúde da população, garantindo a segurança sanitária

de produtos e serviços, participando da construção de seu acesso (BRASIL, 1999).

A Resolução da Diretoria Colegiada (RDC) da Anvisa de nº 48 de 16 de Março de

2004 define fitoterápico como sendo o medicamento obtido empregando-se exclusivamente

matérias-primas ativas vegetais. É caracterizado pelo conhecimento da eficácia e dos riscos de

seu uso, assim como pela reprodutibilidade e constância de sua qualidade. Esse regulamento

abrange medicamentos cujos princípios ativos são derivados de droga vegetal, ou seja,

produtos de extração da matéria-prima vegetal (extrato, tintura, óleo, cera, exsudato, suco

entre outros). Não sendo objeto de registro ou cadastro a planta medicinal ou as suas partes

após os processos de coleta, estabilização e secagem, podendo ser íntegra, rasurada, triturada

ou pulverizada (BRASIL, 2004). Desse modo, a diferença entre planta medicinal e

fitoterápico reside no processo de elaboração que a planta sofre. Quando a elaboração

privilegia uma formulação específica, é caracterizado como um fitoterápico (MARQUES,

2009).

As plantas medicinais têm se mostrado importantes para a medicina moderna.

Primeiramente, por poder fornecer fármacos que dificilmente seriam obtidos por síntese

química, como os alcalóides de Papaver somniferum e os glicosídeos cardiotônicos de

Digitalis spp. As fontes naturais ainda fornecem compostos que podem ser modificados,

tornando-os mais eficazes ou menos tóxicos. E por último, os produtos naturais podem ser

utilizados como protótipos para obtenção de fármacos com atividades terapêuticas

semelhantes a dos compostos originais (ROBBERS et al., 1996). Porém, essas vantagens não

comprovam a efetividade e nem a ausência de toxicidade dessas plantas, faz-se necessário

estudos que garantam sua eficácia e segurança (TUROLLA, NASCIMENTO, 2006).

Muitas plantas contêm substâncias capazes de exercer ação tóxica sobre organismos

vivos. Essas substâncias seriam formadas com a função de defender a espécie dos seus

predadores. Por isso, muitas delas podem estar acumuladas e serem de elevada toxicidade

como os glicosídeos cianogênicos presentes na mandioca-brava, proteínas tóxicas como a

ricina contidas na mamona, alcalóides como a coniina presentes na cicuta e a estricnina, na

noz-vômica. Cabe ressaltar que enquanto algumas plantas são completamente desconhecidas

quanto ao seu potencial tóxico, outras já são comumente reconhecidas a exemplo das

popularmente conhecidas como comigo-ninguém-pode, coroa-de-cristo, pinhão-de-papagaio,

aroeira brava, mamona e cartucheira (SIMÕES et al., 2004).

A toxicidade de uma substância é definida como sendo a capacidade de causar dano

grave ou morte de um determinado organismo (DRAIZE et al., 1944). Em potencial, toda

substância é capaz de produzir efeitos tóxicos, dependendo apenas de fatores como dose,

tempo, modo e freqüência de administração (LOOMIS; HAYES, 1996). A obtenção de dados

toxicológicos em humanos é bastante limitada devido às questões éticas, morais e legais.

Portanto, a maioria desses dados é obtida por meio de testes pré-clínicos que utilizam animais

de laboratório em condições padronizadas (BOELSTERLI, 2003).

2.3. Toxicologia pré-clínica de plantas medicinais

No Brasil, o estudo toxicológico pré-clínico de plantas medicinais é regulamentado

pela RDC nº 90 de 16 de Março de 2004 que norteia alguns ensaios de toxicidade aguda,

toxicidade de doses repetidas e de longa duração (trinta dias a um ano, a depender da

freqüência de uso do produto) (BRASIL, 2004). Esses ensaios têm por objetivo comprovar a

eficácia e a toxicidade das plantas visando os seus benefícios para o ser humano. Além disso,

traçam o perfil dos efeitos colaterais, relacionando-os às doses e a um possível mecanismo de

ação em várias espécies de animais de experimentação (SIMÕES et al., 2004).

A toxicidade aguda diz respeito aos efeitos tóxicos decorrentes da administração da

substância, em doses única ou múltiplas, por um período de 24 horas, e também seus efeitos

durante 14 dias após a administração (LARINI; OLIVEIRA, 1993). Nesse estudo, as

informações obtidas devem ser usadas em estudos subsequentes de toxicidade prolongada

(LOOMIS; HAYES, 1996).

A toxicidade prolongada analisa e caracteriza todos os efeitos provocados por uma

determinada substância quando administrada diariamente em animais experimentais, em

doses previamente estabelecidas e por longos períodos (LOOMIS; HAYES, 1996). Os

principais parâmetros a serem observados durante o tratamento são as modificações na

atividade motora e comportamental, na cor e textura dos pêlos e na frequência

cardiorespiratória. Ao final dos experimentos, os animais devem ser sacrificados para a coleta

do sangue (análise dos parâmetros bioquímicos e hematológicos) e remoção de órgãos para as

análises macro e microscópica (BARNES; DOURSON, 1988).

2.3.1. Toxicologia do desenvolvimento: teratologia e toxicologia da reprodução

A toxicologia do desenvolvimento engloba a teratologia e a toxicologia da reprodução.

A teratologia estuda as alterações induzidas durante o desenvolvimento, entre a concepção e o

nascimento. Enquanto que a toxicologia da reprodução estuda os efeitos adversos que

ocorrem nos sistemas reprodutores masculino e feminino resultantes da exposição aos agentes

químicos (BARROS; DAVINO, 1996).

Os maiores e mais sérios riscos associados à exposição de drogas durante a gestação

consistem na possibilidade de malformações congênitas, retardo no desenvolvimento e

embriofetoletalidade. Os agentes capazes de promover malformações congênitas são

chamados de teratogênicos, enquanto àqueles que produzem retardo no desenvolvimento,

variações na ossificação ou morte do concepto, mas não promovem malformações são

conhecidos como embriofetotóxicos (WEBSTER; FREEMAN, 2001).

Os efeitos deletérios das drogas podem ser promovidos pela ação direta sobre o feto ou

indiretamente, através da ação tóxica sobre o organismo materno (MENEGOLA et al., 1998).

O impacto e a gravidade desses efeitos estão diretamente relacionados com o período da

gestação, a dose ou magnitude da exposição, diferenças entre as espécies, fatores genéticos e

toxicocinéticos como metabolismo, transporte placentário e via de administração

(WEBSTER; FREEMAN, 2001).

Os parâmetros toxicocinéticos são fundamentais para determinar o grau de

embriofetotoxicidade de um xenobiótico. A placenta, por ser uma estrutura responsável pela

troca de substâncias entre mãe e concepto, tem importância vital quando se quer avaliar os

riscos de uma substância. A placenta atua como uma barreira seletiva, é capaz de sintetizar e

metabolizar substâncias, mas pode funcionar como um reservatório de drogas. Alterações na

estrutura e função placentárias promovidas por determinadas substâncias podem produzir

necrose, reduzir o fluxo sanguíneo ou inibir a passagem de nutrientes pela placenta (SILVA et

al., 2009).

De acordo com a sensibilidade aos agentes teratogênicos e embriofetotóxicos, a

gestação pode ser dividida em três fases: a primeira, conhecida como pré-implantação,

compreende o período que vai desde a fecundação até a implantação do blastocisto. Nos seres

humanos, esse período vai até o 17º dia de gestação, enquanto em ratos vai até o 6º dia,

aproximadamente. A segunda fase, a organogênese, na espécie humana vai do 18º ao 40º dia

de gestação e nos ratos, do 7º ao 14º dia. A última fase, conhecida como período fetal, vai até

o término da gestação que nos ratos dura em média do 15º ao 21º dia (ALMEIDA et al.,

2000).

No período de pré-implantação, o embrião é bastante resistente à teratogênese,

encontra-se com células totipotentes em divisão, sem que haja acréscimo citoplasmático

(FRITZ; GIESE, 1990; ALMEIDA et al., 2000). Após a implantação, o embrião inicia o

período de organogênese, que é caracterizado por uma intensa proliferação e migração

celular, remodelamento tissular e formação rudimentar das estruturas do corpo. É o período de

maior susceptibilidade à ação de agentes teratogênicos e embriofetotóxicos, no qual o maior

número de malformações pode ser induzido (BRENT et al., 1993). O período fetal tem início

com o fim da organogênese e é caracterizado por diferenciação e crescimento tissular,

maturação fisiológica dos diferentes sistemas e crescimento ponderal do feto (FRITZ; GIESE,

1990). A sensibilidade às malformações anatômicas durante essa fase é baixa, porém a

exposição aos agentes químicos pode produzir morte celular, inibição da diferenciação e da

divisão celular, interferência na formação dos sistemas nervoso, endócrino e imunológico,

promovendo desordens funcionais e de comportamento (AROUX, 1997).

Há várias plantas que tiveram sua toxicidade reprodutiva comprovada através de

estudos em roedores. Das sementes de Momordica charantia L. foram isoladas glicoproteínas

(alfa e beta- momorcharina) com ação abortiva em camundongos e ação inibitória sobre a

multiplicação celular no endométrio e miométrio (CHAN et al., 1985). Ruta graveolens L.,

conhecida como arruda, demonstrou uma atividade antifertilidade para as suas partes aéreas

moídas e seus extratos aquosos (GANDHI et al., 1991). A administração de diferentes

extratos de Jatropha curcas L. em ratas prenhes acarretou reabsorção fetal nas mesmas

(GOONASEKERA et al., 1995). De Peumus boldus Mol., o boldo-do-chile, as ações abortiva

e teratogênica foram evidenciadas para o extrato hidroalcóolico das suas folhas secas e

também para o alcalóide boldina, considerado um dos seus principais componentes

(ALMEIDA et al., 2000).

Avaliar a possível toxicidade reprodutiva da resina de Aloe ferox Miller em ratas

Wistar prenhes e em seus conceptos torna-se relevante, uma vez que a planta é

tradicionalmente usada no tratamento da constipação intestinal (HALLER, 1990;

BRUNETON, 1995). Considerando-se que a constipação é um problema digestivo bastante

freqüente no período gestacional (CAPELHUCNICK et al., 2008) e que muitas gestantes

fazem uso de plantas medicinais como importante forma de terapia (WEIER, BEAL, 2004), o

estudo de segurança de uso dessa planta é um requisito importante para os testes de avaliação

de um provável fitomedicamento.

3.0. Objetivos

3.1. Objetivo Geral

Avaliar a toxicidade reprodutiva pré-clínica da resina de Aloe ferox Miller sobre a

fertilidade, prenhez e desenvolvimento pós-natal da prole de ratas Wistar.

3.2. Objetivos Específicos

• Identificar a aloína presente na resina por cromatografia em camada delgada e

quantificar os derivados hidroxiantracênicos, expressos como aloína, por

espectrofotometria na luz ultravioleta.

• Analisar o efeito do tratamento, por via oral, da resina de Aloe ferox sobre os períodos

de pré-implantação e organogênese da prenhez de ratas Wistar e verificar as

consequências desse tratamento nos conceptos.

• Investigar o efeito da administração subcrônica, por via oral, da resina de Aloe ferox

durante toda a prenhez de ratas Wistar e sobre o desenvolvimento comportamental da

prole.

Reproductive toxicological study of Aloe ferox Miller (Liliaceae) resin in

female Wistar rats

Ethnopharmacological relevance: Aloe ferox Miller resin in powder form has been used in folk medicine as stomachic, emmenagogue, anthelmintic and laxative. Aim of the study: To identify and quantify the aloin present in the resin and evaluate the effects of Aloe ferox on the fertility, pregnancy and offspring postnatal development of female Wistar rats. Materials and

Methods: The resin in powder form was dissolved in vehicle (glycerin solution 40% - v/v). The study was conducted in three phases, each one containing 5 groups of pregnant rats (n = 8-9/group), in a total of 15 groups of rats randomly formed. The groups 1 and 2 (control groups) received distilled water and vehicle, respectively, while the others were treated orally with A. ferox at doses 0.1, 0.5 and 2.5 g/kg. In the first phase, the treatment was administered from 1st to 6th day (preimplantation period - PP) and the second, from 7th to 14th day (organogenesis period - OP). On day 20th pregnancy, the rats were laparotomized for evaluation of reproductive parameters. In the last phase, the pregnant rats were treated orally with the same doses during whole pregnancy and maternal and offspring parameters were evaluated. Results: the aloin (Rf 0.35) was identified and the percentage of hydroxyanthracene derivates expressed as aloin was 33.5%. During the PP there was reduction in the maternal body mass gain at all doses, with significance level was set at p < 0.05. Reductions were also observed in some fetal and maternal parameters at doses 0.5 and 2.5 g/kg when compared with both control groups. In the OP, the reduction in the maternal body mass gain was observed only at the highest. However, other parameters were changed at all doses evaluated, including the group that received glycerin when compared with water group. The maternal reproductive parameters during whole pregnancy showed increases only in the pups body mass and length on days 7th and 21th postnatal life, respectively, at the highest dose. The presence of a death fetus in this dose suggests a possible toxic effect of this plant when administered to pregnant rats. Conclusion: The neurological and behavioral development of offspring was not influenced by changes in maternal and fetal parameters verified in the treatment with A. ferox. 1. Introduction

Genus Aloe plant has been traditionally applied for the medicinal practice over

thousands of years in many cultures of the world. There are about 300 Aloe species accepted

and used for various medical, cosmetic and nutraceutical purposes (Ni et al., 2004). Aloe ferox

Miller (Liliaceae) is native from South Africa (Zahn et al., 2007). In Brazil, the plant is

popularly known as babosa and the largest producers are in São Paulo (mainly Jarinu city),

Santa Catarina and Northeast region. This region has the best conditions for planting

(Magalhães, 2005). It consists of arborescent perennial shrub that grows in hot and dry

climates. It is made of elongated and pointed leaves and it is separated in two parts: gel and

latex (Berti, 2008). The gel is leave pulp, appears to be clear and mucilaginous, obtained from

parenchymal tissue of inner leave (Tyler, 1994). Chlorophyll parenchyma extract is bitter

yellow latex, commonly termed aloe juice. It is produced in mesophyll excretory cells located

under leaf epidermis (Vogler; Ernst, 1999). Aloe ferox resin is the solid residue obtained by

evaporating latex which drains from the transversally cut leaves (Bruneton, 1995).

In folk medicine, the resin in powder form is used as stomachic, emmenagogue,

anthelmintic and laxative (Balbach, 1986). Various pharmacological and therapeutic activities

of Aloe have been studied and include anti-arthritic (Newcall et al., 1996), anti-inflammatory,

anti-bacterial, anti-viral, laxative, protection against radiation and immunostimulation effects

(Ni et al., 2004); antidiabetic influence (Beppu et al., 2006) and antitumorigenic actions

(Shimpo et al., 2006).

The components of Aloe ferox are primarily glycoproteins, polysaccharides, low–

molecular-weight substances and anthraquinones. Polysaccharides are largely glucomannans.

Galactose and galactouronic acid polymers are also frequently found. Low-molecular-weight

substances are reported, such as aloesin, sitosterol, diethylhexylphthalate, vitamins and beta-

carotene. In addition to these, aloe-specific anthraquinones are also present and include aloe-

emodin, aloin, barbaloin and isobarbaloin (Choi; Chun, 2003). Chlorophyll parenchyma

extract consist of anthraquinone glycosides, mainly aloin. Aloin was characterized as 10-β-D-

glucopyranosyl-1,8-dihydroxy-3-hydroxymethyl-9(10-H)-anthracenone (Hay; Haynes, 1956;

Conner et al., 1989; Yuko et al., 1990). It is a C-glycoside aloe-emodin that is a mixture of

two diastereoisomers: aloin A (also called only aloin) and aloin B (also called barbaloin)

(Haynes, 1960; Tyler, 1994).

Although its use as laxative is widely spread, a reproductive toxicological study of this

plant is necessary because many pregnant women have constipation in the pregnancy

(Capelhucnick et al., 2008) and they use medicinal plants as important form of therapy

(Weier; Beal, 2004). Therefore, this study was conducted to investigate the effects of the oral

administration of Aloe ferox resin in pregnant rats Wistar, during preimplantation and

organogenesis periods, whole pregnancy and their offspring postnatal development.

2. Materials and methods

2.1. Plant material

Aloe ferox resin used in the current study was a commercial preparation produced and

kindly donated by Odaly Soares Laboratory, Ceará, Brazil. Aloe ferox resin is soluble in

alkalis, concentrated acetic acid, absolute alcohol, glycerin and it is partially soluble in water

(Brazilian Pharmacopoeia, 1988). The glycerin was the solvent that showed better solubility

and compatibility with in vivo experiments. In the laboratory, the resin was grinded down in

automatic processor and solubilized in glycerin a.p (Vetec®) 40% (v/v).

2.2. Animals

Pregnant Wistar rats (Rattus norvegicus), aged 3 months and weighing 200 – 240 g,

were obtained from the Department of Physiology and Pharmacology at the Federal

University of Pernambuco (UFPE). They were maintained in standard environmental

conditions (22 ± 2ºC, 12:12h dark/light cycle). Water and industrialized dry food (Labina®,

Purina, Brazil) were available ad libitum. The experimental protocol was approved by the

Animal Experimentation Ethic Committee of UFPE (Process nº 23076.007267/2009-96), in

accordance with the National Institute of Health Guide for the Care and Use of Laboratory

Animals.

2.3. Experiments

2.3.1. Identification of aloin by thin layer chromatography

To 0.25 g of the powdered sample added 20 mL of methanol and 10 µL of this

solution were applied silica gel plates (Merck-Alemanha art. 105554). Developed over a bath

using a mixture of water-methanol-ethyl acetate (13:17:100 v/v). Allowed the plate to dry in

air, sprayed with 100 g/L solution of potassium hydroxide in methanol, the retention factor

(Rf) was obtained and the plate was examined in ultraviolet light at 365 nm. It was used an

aloin solution as reference standard (European Pharmacopoeia, 1997).

2.3.2. Quantification of hydroxyanthracene derivates by spectrophotometry

The extractive solution was obtained by heating 0.4 g of powdered sample with

established volume of methanol, water, ferric chloride (600 g/L) and hydrochloric acid (a.p.)

solutions. The heating occurred in a water-bath under a reflux condenser for 4h. Allowed to

cool, transferred the solution to a separating funnel, ether was added, discarded the aqueous

phase and then was obtained an UV absorption spectrum from the organic phase, in alkaline

medium, at the wavelength range from 200 to 600 nm (50 Uv-Vis Spectrophotometer). The

specific absorbance of aloe-emodin, which is obtained by oxidation of aloin, is 512 nm. The

methanol was used as the compensation liquid. The percentage of hydroxyanthracene

derivates expressed as aloin was calculated from the expression (European Pharmacopoeia,

1997):

2.3.3. Experimental protocol

Observation of the presence of sperm in the vaginal smear was used to establish the

1st day of pregnancy. The study was conducted in three phases, each one containing five

groups of pregnant rats randomly formed (n = 8-9/group), in a total of 15 groups. The groups

1 and 2 (control groups) received distilled water and glycerin solution 40% (v/v) (1 mL, p.o.),

respectively, while the others were treated orally with A. ferox at doses 0.1, 0.5 and 2.5 g/kg

in glycerin 40% (v/v). Aloe ferox doses were based on a previous one-year toxicity study of

Aloe arborescens Miller in rats, which used diets containing until 2.7 g/kg of Aloe (Matsuda

et al., 2008). In the first phase, the treatment was administered from 1st to 6th day

(preimplantation period), the second, from 7th to 14th day (organogenesis period) and the

third, from 1st to 21th day (whole pregnancy). During pregnancy, the rats were evaluated for

survival, altered appearance and any clinical signs of toxicity, such as changes in food and

water intake, piloerection, diarrhea, change of locomotor activity and vaginal bleeding.

Parts 1 and 2: in the preimplantation and organogenesis periods, the maternal weight

was recorded on days 1, 7, 14 and 20. On the 20th day of pregnancy, the rats were

anesthetized with Pentobarbital (35 mg/kg i.p.), laparotomized and their uterine horns

removed. The number of implants, resorption, live and death fetuses were then recorded.

Ovaries and placentae were weighed and the corpora lutea were counted. The fetuses were

weighed, measured and observed for any visible abnormalities. From these data, the

implantation index (total number of implantation sites/total number of corpora lutea × 100),

the preimplantation loss rate (number of corpora lutea - number of implantations/number of

corpora lutea × 100) and the postimplantation loss rate (number of implantations - number of

live fetuses/number of implantations × 100) were calculated (Costa-Silva et al., 2007).

Part 3: during whole pregnancy, the maternal body weight was also recorded on days

1, 7, 14 and 21. Pregnant rats were allowed to complete their pregnancy to term. After birth,

macroscopic aspects were observed in order to detect possible fetal malformations and the

following reproductive parameters were analyzed: offspring/dam ratio, days of pregnancy,

pregnancy index (% pregnant rats with all live pups), viability index (number of live pups on

day 4 of postnatal life/number of live offspring born × 100) and lactation index (number of

live pups on day 21 of postnatal life / number of live pups on day 4 of postnatal life × 100).

The pups body weight and length were determined on days 1, 4, 7, 14 and 21 of postnatal life

(Silva et al., 2009).

After birth, one-third offspring from control and treated groups (0.1, 0.5 and 2.5 g/kg)

was analyzed to the following behavioral parameters: postural reflex, eye-opening day, adult

walking day and spontaneous ambulation. The pups postural reflexes were evaluated on 1st

and 7th days of life. The pups were put on plane surface, in supine position, and the righting

reflex was measured in seconds. The eye-opening day was determined by observation of

partial displacement of the palpebral fissure at least one eye. It was considered adult walking

day when pups ambulated without dragging hind feet and without touching belly on the floor.

The spontaneous ambulation was determined on 20th day postnatal life using a square

measuring 30 x 30 cm and divided in nine equal spaces. The pup was put on the central part

and during two minutes was recorded the number of invaded square when it put at least three

feet on this area (Costa-Silva et al., 2006).

2.4. Statistical analysis

Values were expressed as mean ± standard error of mean (SEM) or as median. The

differences between treated and control groups were analyzed by variance analysis

(ANOVA), followed, when necessary, by the Tukey test. The implantation index, pre- and

postimplantation loss rates were analyzed using the Kruskal-Wallis test followed by the Dunn

test, when necessary. The pregnancy, viability and lactation indexes were analyzed using

Fisher’s exact test. The significance level was set at p < 0.05. Statistical analyses were

performed with the aid of the computer program GraphPad Prism 5.0.

3. Results

The chromatogram obtained with the solution test showed in the central part a zone of

yellow fluorescence (aloin – Rf 0.35) similar in position to the zone corresponding to aloin in

the chromatogram obtained with the reference solution. The percentage content of

hydroxyanthracene derivates expressed as aloin was 33.5% and its absorption spectrum is

showed in Figure 1.

During the treatment period with A. ferox no deaths of pregnant rats was registered

and any clinical signs of toxicity, such as piloerection, change of locomotor activity and

vaginal bleeding were observed. A mild diarrhea was recorded mainly at dose 2.5 g/kg.

In the preimplantation period, no statistical difference between the groups treated with

glycerin and water was observed. However, significant reductions were found in maternal

body mass gain in this period (at all doses), in the mass and length of the fetuses and absolute

mass of the placenta (at dose 2.5 g/kg) when compared to both control groups. The relative

mass of the fetuses (at dose 0.5 g/kg) and the relative mass of the placenta (at doses 0.5 g/kg

and 2.5 g/kg) showed also significant reductions when compared with glycerin group. At dose

0.5 g/kg, the reduction in length of the fetuses was significant in relation to water group (table

1).

In the organogenesis period, significant reductions were observed in the maternal body

mass gain in this period (at dose 2.5 g/kg) when compared with glycerin group. At dose 0.5

g/kg, there was an increase in the absolute mass of fetuses when compared with water group.

The absolute mass of the placenta (at dose 0.1 g/kg), the relative mass of the placenta (at all

doses including glycerin group) and the relative mass of the ovary (at dose 2.5 g/kg) were

statistically lower than group treated with water (table 2).

In both pregnancy studies, no death fetuses or fetuses with external malformations

were observed. No statistically significant differences were found in the reproductive

variables: offspring/dam relationship, ovary absolute mass and number of corpora lutea. There

were also no significant differences in implantation index, pre and postimplantation losses

rates between the groups treated and their respective controls (tables 1 and 2).

The maternal reproductive parameters during whole pregnancy showed increases only

in the pups body mass and length on days 7th and 21th postnatal life, respectively, at the

highest dose when compared with glycerin group. Only one death fetus was found at dose 2.5

g/kg (table 3). There was increase in the food intake at dose 0.5 g/kg when compared with

glycerin group (Figure 2), but no significant difference was found in the maternal body mass

gain in this period (Figure 3). An increase in the water intake was observed at doses 0.5 and

2.5 g/kg when compared with both control groups and at dose 0.1 g/kg, when compared only

glycerin control (Figure 4). After birth of offspring, it was not observed external abnormalities

and their behavioral parameters were not changed when compared with control groups (table

4).

4. Discussion

The aloin (Rf 0.30-0.35) gives yellow fluorescence when examined in UV light at 365

nm (European pharmacopoeia, 1997) and the A. ferox resin contains not less than 18% of

hydroxyanthracene derivates expressed as aloin (European Pharmacopoeia, 1997) and it can

reach until 40% (Costa, 1975). It was responsible for mild diarrhea observed mainly in rats

treated with highest dose and this explains the increase in the water intake.

After oral administration, the aloin is not absorbed in the upper intestine, it is

hydrolyzed in the colon by Eubacterium sp. and then reduced to the active metabolite aloe-

emodin anthrone. It stimulates colonic motility, augmenting propulsion and accelerating

colonic transit, which reduces fluid absorption from the fecal mass (Bradley, 1992; Akao et

al., 1996). It also increases paracellular permeability across the colonic mucosa probably

owing to an inhibition of Na+-K+-ATPase or to an inhibition of chloride channels, which

results in an increase in the water content in the large intestine (Witte, 1993).

The glycerin used as solvent for A. ferox resin is low toxicity and the LD50 for rats is

12.6 g/kg (p.o.). Even taking laxative action (Portantiolo, 2007), it did not cause diarrhea in

the animals treated only with glycerin 40% (v/v) solution and nor affected the reproductive

parameters of pregnant rats.

This study showed that the treatment with Aloe ferox can change some maternal and

fetal parameters during the preimplantation and organogenesis periods, but it did not cause

deaths of pregnant rats and fetal malformations. In the preimplantation period, the embryo is a

mass of undifferentiated cells in mitotic division, because of this it is quite resistant to

teratogenesis (Hodgen; Itskovits, 1988; Almeida et al., 2000). The organogenesis period is

characterized by intense cellular proliferation and migration, tissue remodeling and body

structure formation (Brent et al., 1993). The type of malformation depends on the

evolutionary stage of the embryo and the affinity of the teratogenic agent by the embryonic

tissue (Chang et al., 2002). As the pre and postimplantation losses in the treated groups were

not significantly differents from the control groups, it can be assumed that the number of

blastocysts implanted and embryonic development were similar between groups.

The treatment during whole pregnancy did not interfere in the offspring normal

development. The behavioral parameters analyzed in this study were used to verify possible

deleterious actions of A. ferox on behavioral and neurological development of the offspring

(Carlini et al., 1988). The presence of one death fetus suggests a possible abortive effect of A.

ferox at dose 2.5 g/kg. Aloe arborescens was verified that it can stimulate uterine smooth

muscle contraction and can cause abortion in pregnant woman (Belew, 1999). The no

observed adverse effect level (NOAEL) for A. arborescens was estimated to be 109.7

mg/kg/day in females rats (Matsuda et al., 2008), but in this study the NOAEL was lower than

100 mg/kg/day, since adverse effects already appear in this dose.

In conclusion, the parameters changed by treatment with A. ferox did not cause deaths

of pregnant rats, fetal malformations and nor compromised the offspring normal development.

The oral administration of A. ferox could induce some maternal toxicity mainly at the highest

dose due to possible abortive effect shown in this dose. This suggests that further studies

should be conducted to ensure the use of the plant during human gestation.

5.0. Conclusão

Os resultados obtidos permitem constatar os seguintes aspectos:

- A identificação da aloína presente na resina de Aloe ferox como também a porcentagem de

derivados hidroxiantracênicos expressos como aloína condizem com os dados da literatura.

- O efeito laxante, observado principalmente no tratamento com a maior dose, deve-se à

presença de glicosídeos antraquinônicos na resina de A. ferox, justificando seu uso na

medicina popular.

- A utilização da solução de glicerina 40% (v/v) como solvente da resina pulverizada de A.

ferox não interferiu nos parâmetros reprodutivos durante os períodos de pré-implantação,

organogênese e toda a prenhez das ratas Wistar como também não comprometeu o

desenvolvimento pós-natal de sua prole.

- A administração oral da resina de A. ferox nas ratas prenhes não evidenciou sinais clínicos

de toxicidade, não ocasionou a sua morte, não interferiu na progressão da prenhez das

mesmas e os parâmetros reprodutivos que foram alterados pelo tratamento, não

comprometeram o desenvolvimento normal da prole.

- A presença de um natimorto, na dose de 2,5 g/kg, sugere um possível efeito tóxico de A.

ferox quando administrado em ratas prenhes. Porém, estudos posteriores são necessários para

garantir o uso dessa planta durante a gestação humana
