NOTA PRÉVIA
O Relatório Anual de Segurança na Aviação Civil é elaborado e divulgado em con-
formidade com o Regulamento (UE) n.º 996/2010, do Parlamento Europeu e do
Conselho, de 20 de outubro de 2010 e pretende-se com a sua elaboração e divulga-
ção, numa vertente preventiva, apoiar a comunidade aeronáutica na gestão da
segurança operacional e na identificação de áreas críticas bem como outros aspe-
tos relevantes, com base na análise dos acidentes e incidentes ocorridos nos últi-
mos cinco anos e, em casos pontuais, a sua comparação com o analisado nos últi-
mos dez anos.
Os dados que seguidamente serão apresentados foram recolhidos pela equipa do
GPIAAF com base nas informações rececionadas neste Gabinete e destinam-se,
exclusivamente, à prevenção de acidentes e incidentes na aviação civil e à divulga-
ção de informação geral, não devendo ser utilizados para outros fins, e não têm
como finalidade atribuir culpas ou responsabilidades, destinando-se exclusivamen-
te a apresentar a descrição dos eventos e a análise das ocorrências com a finalida-
de única de prevenir futuros acidentes e incidentes, contribuindo assim para a
segurança operacional na aviação civil, em conformidade com o Regulamento (UE)
n.º 996/2010 e o Anexo 13 da Convenção sobre a Aviação Civil Internacional.
Nos termos da Resolução do Conselho de Ministros 2/2018, e das regras de mo-
dernização administrativa e das boas práticas implementadas no GPIAAF, este
relatório é apresentado, exclusivamente, em formato digital e, pela primeira vez
em formato bilingue.
PRELIMINARY NOTE
The Annual Safety Report on Civil Aviation was elaborated and disseminated in
accordance with Regulation (EU) No 996/2010, of the European Parliament and the
Council of 20 October 2010 and it is projected. in a preventive aspect, to support
the aeronautical community in the management of operational safety and in the
identification of critical areas as well as other relevant aspects, based on the analy-
sis of accidents and incidents that have occurred within the last five years and, in
specific cases, their comparison with that analysed in the last ten years.
The following data has been collected by the GPIAAF team based on the infor-
mation received from them and is intended exclusively for the prevention of acci-
dents and incidents in civil aviation and the dissemination of the information,
should not be used for other purposes, and is not intended to assign faults or re-
sponsibilities, its intent is solely to provide a description of events and the analysis
of occurrences for the sole purpose of preventing future accidents and incidents,
thus contributing to operational safety in civil aviation, in accordance with Regula-
tion (EU) No 996/2010 and Annex 13 to the Convention on International Civil Avia-
tion.(ICAO).
In accordance with Portuguese Council of Ministers Resolution 2/2018, and the
guidelines for administrative modernisation and good practices implemented at
GPIAAF, this report is presented exclusively in digital format and for the first time in
bilingual format.

INTRODUÇÃO
Os dados recebidos no GPIAAF e apresentados neste relatório, são determinantes
para a prevenção de futuras ocorrências e devem merecer a maior atenção do seu
destinatário principal, a comunidade aeronáutica.
Com a adoção, por parte de alguns operadores, da introdução das notificações no
repositório europeu, notificando o GPIAAF apenas os casos que classificam como
Acidente ou Incidente Grave, o número de notificações submetidas ao GPIAAF
reduziu substancialmente, tendo contribuído também para essa redução, a situa-
ção pandémica que originou uma limitação drástica no tráfego aéreo.
Contudo, a comunidade aeronáutica deve manter a boa prática do reporte de ocor-
rências relevantes para a segurança, em cumprimento da legislação aplicável nesta
matéria, não só no âmbito do reporte obrigatório, mas também no que respeita ao
reporte voluntário, o qual nunca é demais encorajar. Apenas deste modo será pos-
sível identificar tendências de forma fiável, que permitam maximizar a aprendiza-
gem por todos os envolvidos, contribuindo para a melhoria da segurança da avia-
ção civil.
O presente relatório é constituído, essencialmente, por três capítulos:
I - O GPIAAF e o seu papel na aviação civil – Missão, atribuições e recursos.
II - A Investigação de segurança em 2020 com a análise da atividade de investigação
de segurança de acidentes e incidentes com aeronaves realizada nesse ano pelo
GPIAAF.
III - A avaliação comparativa da evolução da atividade de investigação de segurança
de acidentes e incidente com aeronaves, no período entre 2016 e 2020, assim co-
mo a estatística extraída dos eventos investigados por se considerar pertinente
expor e analisar a sua linha temporal evolutiva.
INTRODUCTION
The information received at GPIAAF and presented in this report is crucial for the
prevention of future occurrences and should deserve the greatest attention of the
aeronautical community.
With the implementation, by some operators, of the submission of notifications in
the European repository, notifying GPIAAF only in cases that classify as Accident or
Serious Incident, the number of notifications submitted to GPIAAF has decreased
substantially. The pandemic situation which has led to a drastic limitation in air
traffic has also contributed to this reduction,
However, the aeronautical community must maintain the good practice of report-
ing safety-relevant occurrences in compliance with the applicable legislation in this
area, not only in the context of compulsory reporting, but also regarding voluntary
reporting, which is never too much to encourage. Only in this way will it be possible
to identify trends reliably, which will enable everyone involved to increase lessons
learning, contributing to the improvement of civil aviation safety.
This report consists essentially of three chapters
I - GPIAAF and its role in civil aviation - Mission, assignments, and resources.
II - Safety Investigation during 2020 with the analysis of accident safety investiga-
tion activities and aircraft incidents carried out by the GPIAAF.
III - The comparative evaluation of the activity of accident safety investigation and
aircraft incident, in the period between 2016 and 2020, as well as the statistics
obtained from the events investigated because it is considered pertinent to expose
and analyse its evolutionary timeline

MISSÃO
O GPIAAF tem por missão investigar os acidentes e incidentes relacionados com a
segurança das aeronaves civis e dos transportes ferroviários, visando a identifica-
ção das respetivas causas com o objetivo de contribuir para a prevenção de futuros
acidentes e incidentes. Funciona de modo independente, na sua organização, es-
trutura jurídica e processo de decisão, das autoridades responsáveis pela seguran-
ça, das entidades reguladoras da aviação civil e do transporte ferroviário, de qual-
quer gestor de infraestrutura, empresa de aviação civil, empresa ferroviária, e de
qualquer outra parte cujos interesses possam colidir com as tarefas que lhe são
confiadas.
VISÃO
Tendo como premissa a Missão, foi definida como Visão, contribuir de forma de-
terminada para o avanço da segurança na aviação civil e no transporte ferroviário,
exercendo as melhores práticas de investigação de acidentes e incidentes, em de-
fesa do interesse público.
A UNIDADE DE AVIAÇÃO CIVIL
Em conformidade com os compromissos internacionais assumidos na Convenção
de Chicago sobre Aviação Civil Internacional, assinada em Chicago, em 7 de dezem-
bro de 1944, nomeadamente, os estabelecidos no seu Anexo 13, Portugal está
obrigado a investigar os acidentes e incidentes com aeronaves civis com a finalida-
de exclusiva da prevenção de acidentes.
MISSION
The mission of GPIAAF is to investigate accidents and incidents related to the safety
of civil aircraft and rail transport, with the purpose of identifying their causes to
contribute to the prevention of future accidents and incidents. It operates inde-
pendently, in its organisation, legal structure and decision-making, of the safety
authorities, civil aviation and rail regulatory authorities, of any infrastructure man-
ager, civil aviation company, railway company, and of any other whose interests
may conflict with the goals assigned to it.
VISION
Based on the principles of its Mission, GPIAAF envisions to contribute in a decisive
way to the advancement of safety in civil aviation and rail transport, exercising the
best practices of investigation of accidents and incidents, in defence of the public
interest.
THE CIVIL AVIATION UNIT
In accordance with the international commitments made in the Chicago Conven-
tion on International Civil Aviation, signed in Chicago on December 7th, 1944, name-
ly the ones set in Annex 13, Portugal is required to investigate accidents and inci-
dents involving civil aircraft for the sole purpose of accident prevention.

Esta obrigação do Estado Português foi reforçada pelo Regulamento (UE) n.º
996/2010, do Parlamento Europeu e do Conselho, de 20 de outubro de 2010, rela-
tivo à investigação e prevenção de acidentes e incidentes na aviação civil, cujo
desígnio é o de reforçar a segurança operacional da aviação, através da garantia de
elevados níveis de eficácia, de celeridade e de qualidade nas investigações de aci-
dentes e incidentes graves no âmbito da aviação civil europeia.
ATRIBUIÇÕES
As atribuições do GPIAAF, no âmbito da Aviação Civil, desenvolvidas de modo inde-
pendente das autoridades, aeronáuticas ou outras, cujos interesses possam confli-
tuar com a missão que lhe é atribuída, integram:
A investigação dos acidentes e incidentes com aeronaves civis, com a finalidade
de determinar as suas causas e formular recomendações que evitem a sua re-
petição;
A participação nos programas e políticas de prevenção de acidentes, bem como
a participação em comissões ou atividades, nacionais ou estrangeiras em maté-
ria de prevenção e/ ou investigação;
A promoção de estudos e proposta de medidas de prevenção que visem reduzir
a ocorrência de acidentes e incidentes na aviação civil e, consequentemente, a
sinistralidade aeronáutica;
A elaboração e divulgação dos relatórios técnicos sobre acidentes e incidentes
graves, em conformidade com o artigo 25.º da Convenção de Chicago sobre
Aviação Civil Internacional;
This obligation of the Portuguese State has been strengthened by Regulation (EU)
No 996/2010 of the European Parliament and the Council of October 20th, 2010,
on the investigation and prevention of accidents and incidents in civil aviation,
whose design is to enhance the operational safety of aviation by ensuring high
levels of effectiveness, celerity, and quality in investigations of serious accidents
and incidents in European civil aviation.
REMIT
The tasks of GPIAAF, in the field of Civil Aviation, are developed independently of
the authorities, aeronautical or others, whose interests may conflict with the mis-
sion assigned to it, and include:
The investigation of accidents and incidents involving civil aircraft, with the
purpose of determining their causes and making recommendations to prevent
their repetition;
The participation in accident prevention programmes and policies, as well as
participation in national or foreign commissions or activities in prevention
and/or investigation;
The promotion of studies and the proposal of prevention measures aimed at
reducing the occurrence of accidents and incidents in civil aviation and, conse-
quently, aeronautical accidents;
The preparation and dissemination of technical reports on accidents and seri-
ous incidents in accordance with Article 25 of the Chicago Convention on Inter-
national Civil Aviation;

A participação nas atividades desenvolvidas a nível de organizações internacio-
nais no domínio da investigação e prevenção aeronáuticas;
A organização e divulgação de informação relativa à investigação e prevenção
de acidentes e incidentes com aeronaves civis;
A elaboração de propostas para adequar a legislação às necessidades nacionais
e aos compromissos assumidos internacionalmente nas matérias respeitantes
aos seus objetivos;
A colaboração com os organismos de segurança dos operadores aéreos, dos
serviços de tráfego aéreo e com as associações profissionais nacionais, em ma-
térias de prevenção;
A colaboração com homólogos de outros países na investigação e prevenção de
acidentes e incidentes graves com aeronaves civis;
A preparação, organização e divulgação estatísticas de segurança operacional.
RECURSOS FINANCEIROS
Para o ano de 2020, o plafond inicial atribuído a este Gabinete foi de 1.207.876 €.
Na sequência da publicação da Lei n.º 2/2020, de 02 de março (Orçamento do Es-
tado para 2020), o orçamento efetivamente disponível para o ano de 2020 foi de
1 075 777€.

tions in the field of aeronautical research and prevention;
The organization and dissemination of information on the investigation and
prevention of accidents and incidents with civil aircraft;
The preparation of proposals to adapt legislation to national needs and interna-
tional commitments in matters relating to its objectives;
The collaboration with air operator safety organisations, air traffic services and
national professional associations in prevention matters;
The collaboration with counterparts from other countries in the investigation
and prevention of accidents and serious incidents with civil aircraft;
The preparation and dissemination of operational safety statistics.
FINANCIAL RESOURCES
For the year 2020, the initial value assigned to GPIAAF was €1,207,876. Following
the publication of Law No. 2/2020 of March 2nd (Portuguese State Budget for 2020),
the budget available for the year 2020 was €1,075,777.

RECURSOS MATERIAIS
Para o cumprimento das atribuições que lhe estão acometidas, o GPIAAF ao nível
dos recursos materiais, detém diversos equipamentos, nomeadamente: informáti-
cos; de telecomunicações, audiovisual, gráfico e ferramentas de apoio à investiga-
ção. Detém ainda duas viaturas 4x4 afetas às duas unidades de investigação.
O GPIAAF tem as suas instalações / sede sitas na Praça Duque de Saldanha, 31 – 4.º
em Lisboa e utiliza, ainda, um Hangar no Aeródromo Municipal Gonçalves Lobato,
em Viseu, para acondicionar os destroços das aeronaves acidentadas e proceder à
sua peritagem técnica, nos termos do disposto na alínea e) do n.º 6 do art.º 4.º do
Regulamento n.º 996/2010 de 20 de outubro do Parlamento Europeu e do Conse-
lho, bem como à preservação das provas, nos termos do n.º 1, do art.º 13.º do
referido Regulamento Europeu.
MATERIAL RESOURCES
In order to achieve the goals assigned to GPIAAF, in terms of material resources,
several items are retained, namely: informatics; telecommunications, audio-visual,
graphic and investigation support tools. It also holds two 4X4 vehicles respective to
the two investigation areas.
GPIAAF has its facilities / head office at Praça Duque de Saldanha, 31 - 4th in Lisbon
and also uses a Hangar at the Gonçalves Lobato Municipal Airfield, in Viseu, to
store the wreckage of aircraft and conduct technical expertise, in accordance with
paragraph (e) of Article 4(6) of Regulation No 996/2010 of October 20th of the
European Parliament and the Council, as well as the preservation of evidence in
accordance with Article 13 of Article 13 of that European Regulation.

RESUMO NACIONAL
Apresenta-se, como nota introdutória, o quadro que exibe, de uma forma resumi-
da, o que tem sido o panorama da segurança operacional (safety) e qual a conse-
quente sinistralidade da aviação civil em Portugal verificada no respetivo período
em análise.
Os totais reportam-se ao ano de 2020 e a média anual ao período entre 2016-2020.
NATIONAL SUMMARY
As an introductory note that the diagram below displays, in a summarized way,
what has been the operational safety panorama and the consequent accident rate
of civil aviation in Portugal observed in the respective period under analysis.
The totals are for the year 2020 and the yearly average for the 2016-2020 period.

REPORTES RECEBIDOS
Durante o ano de 2020 foram rececionadas no GPIAAF 2572 notificações iniciais de
eventos com aeronaves inseridas no âmbito do Decreto-Lei n.º 318/99. Neste com-
puto inserem-se as notificações on-line remetidas diretamente ao GPIAAF e, simul-
taneamente para a Autoridade Nacional de Aviação Civil, bem como algumas de
reporte obrigatório e/ou voluntário para a ANAC, que, contudo, não se inserem no
art.º 9º do Regulamento 996/2010, do Parlamento Europeu e do Conselho,
Tendo-se verificado um aumento gradual nos últimos anos do número de notifica-
ções recebidas influenciado pelo aumento do tráfego aéreo associado a um au-
mento da cultura de reporte, em 2020 verificou-se uma redução do número de
notificações rececionadas (gráfico 1) no GPIAAF que se entende dever, essencial-
mente, a dois fatores:
A adoção, por parte de alguns operadores, da introdução das notificações no
repositório europeu;
A situação pandémica mundial que originou uma limitação drástica no tráfego
aéreo.
Após avaliação criteriosa das notificações recebidas no GPIAAF, foram abertos 24
processos de avaliação. destes, foram abertos cinco processos de investigação em
cumprimento do Regulamento (UE) n.º 996/2010 do Parlamento Europeu e do
Conselho, de 20 de outubro, e do Decreto-Lei n.º 318/99, de 11 de agosto.
NOTIFICATIONS RECEIVED
In 2020, 2572 initial notifications of events with aircraft were received by GPIAAF.
This figure includes the online notifications sent directly to GPIAAF and, simultane-
ously to the Portuguese CAA, as well as some mandatory and/or voluntary report-
ing, but which are not part of Article 9 of Regulation 996/2010, the European Par-
liament, and the Council,
The gradual increase in the number of notifications received in the previous years
has been influenced by the increase of air traffic in combination with an increase of
reporting culture, although in 2020 there was a reduction in the number of notifi-
cations received (graph 1) by GPIAAF which was mainly due to the following two
factors:
The adoption by some operators of the submission of notifications directly
into the European repository.
The global pandemic situation which has led to a drastic limitation in air traf-
fic.
After a careful evaluation of the notifications received by GPIAAF, 24 evaluation
processes were launched. Five investigation processes were also launched in com-
pliance with Regulation (EU) No 996/2010 of the European Parliament and the
Council of 20 October and Decree-Law No 318/99 of August 11th.

Foram recebidas, ainda, cinco notificações internacionais tendo o GPIAAF nomeado
representantes acreditados para cooperação naquelas investigações desenvolvidas
por congéneres estrangeiros.
There were also five international notifications received to which GPIAAF appoint-

eign counterparts.

INVESTIGAÇÕES FORMAIS
Na sequência das notificações submetidas ao GPIAAF e após avaliação criteriosa
dos eventos, foram abertos 5 processos de investigação de acidentes. Não se verifi-
cou qualquer evento passível de ser investigado como incidente grave ou incidente
suscetível de trazer ensinamentos em ações preventivas.

Following the notifications submitted to GPIAAF and after careful evaluation of the
events, there was no event that could be investigated as a serious incident or inci-
dent that could bring lessons in terms of preventive actions. However, five accident

INVESTIGAÇÕES CONCLUÍDAS - RELATÓRIOS PUBLICADOS
Durante o ano de 2020 foram concluídas 21 investigações, elaborados e divulgados
os respetivos relatórios finais, reduzindo, assim, o backlog existente de investiga-
ções abertas há mais de dois anos,
Usando como critério de apresentação os acidentes seguidos dos incidentes, am-
bos os tipos por data de ocorrência, apresenta-se seguidamente o resumo dos
processos de investigação concluídos durante o ano em análise bem como os pro-
cessos que, por configurarem causas semelhantes, foram encerrados fazendo a sua
avaliação parte integrante de outros processos de investigação. Todos estes pro-
cessos podem ser lidos na integra através do respetivo link.
INVESTIGATIONS COMPLETED - PUBLISHED REPORTS
During 2020, 21 investigations were completed, the final reports were prepared
and released, thereby reducing the existing backlog of investigations open for more
than two years.
Using as presentation criteria the date of occurrence, the summary of the investi-
gation processes of accidents and incidents with civil aircraft concluded during the
year under analysis as well as the processes that, because they constitute similar
causes, were closed by making their evaluation an integral part of other investiga-
tion processes. All these processes can be read in full via their respective link.

EVOLUÇÃO 2016 – 2020
Este capítulo apresenta elementos estatísticos referentes ao período de cinco anos
compreendido entre 01 de janeiro de 2016 e 31 de dezembro de 2020, os quais
permitem evidenciar a evolução ocorrida nos diversos indicadores e identificar
eventuais tendências.
Inicialmente faz uma introdução à atividade desenvolvida nestes últimos cinco
anos, posteriormente é feita uma avaliação da tipologia de eventos reportados e
analisados que deram origem a investigações de segurança no âmbito da aviação
civil.
O reporte de ocorrências assumiu uma nova diretriz na sequência da publicação do
Regulamento (EU) n.º 376/2014, do Parlamento Europeu e do Conselho com a
criação de um repositório europeu para a comunicação de ocorrências (ECCAIRS2),
obrigatórias e voluntárias. Assim, os reportes de ocorrências de acidentes e inci-
dentes graves, nos termos do Decreto-Lei n.º 318/99 e do Regulamento n.º
996/2010, do Parlamento Europeu e do Conselho, assumem nos registos desta
Autoridade de Investigação, a designação de Reportes SIA (R_SIA).
EVOLUTION 2016 – 2020
This chapter presents statistical elements related to the five-year period between
January 1st, 2016 and December 31st, 2020, which allow us to highlight the evolu-
tion which occurred in the various indicators and identify possible trends.
Initially it shows an introduction to the activity developed over the last five years,
later an evaluation of the type of events reported and analysed that gave rise to
safety investigations in the field of civil aviation.
The report of occurrences took on a new guideline following the publication of
Regulation (EU) No 376/2014, of European Parliament and the Council with the
creation of a mandatory and voluntary European report on a central repository
(ECCAIRS2). Therefore, reports of occurrences of accidents and serious incidents,
pursuant to Decree-Law No 318/99 and Regulation No 996/2010 of the European
Parliament and of the Council, are the name of SIA Reports (R_SIA) in the registers
of this Investigation Authority.

O Gráfico G1 (abaixo) apresenta a evolução dos reportes (obrigatórios e voluntá-
rios) de acidentes e incidentes submetidos ao GPIAAF entre 2016 e 2020. No ano
de 2020 estão destacados os reportes que se inserem no âmbito da atividade desta
Autoridade de Investigação, nos termos do Decreto-Lei n.º 318/99 e do Regula-
mento n.º 996/2010, já anteriormente referidos.
Verifica-se uma tendência crescente decorrente do aumento do tráfego aéreo
associado a uma melhoria na cultura de reporte, a qual sofreu no ano de 2020 um
decréscimo de 23% face ao ano anterior. Considera-se que esta redução espelha
duas realidades: O reporte, por parte de alguns operadores, diretamente no reposi-
tório europeu e, principalmente, a redução drástica dos movimentos aéreos decor-
rentes da pandemia provocada pela COVID-19.
REPORTS RECEIVED
Graph G1 (below) shows the evolution of reports (mandatory and voluntary) of
accidents and incidents submitted to GPIAAF between 2016 and 2020. In 2020, the
reports that fall within the scope of the activity of this Investigation Authority are
highlighted, pursuant to Decree-Law No. 318/99 and Regulation No. 996/2010,
already mentioned above.
There is a growing trend due to the increase in air traffic associated with an im-
provement in the reporting culture, which suffered a decrease of 23% compared to
the previous year in 2020. This reduction is considered to mirror two realities: The
reporting by some operators directly to the European repository and, mainly, the
drastic reduction of air movements resulting from the pandemic caused by COVID-
19.

32 acidentes e 21 incidentes. Pela análise do gráfico verificamos que o ano de 2016
apresentou o maior número de incidentes e o ano de 2019 o maior número de
acidentes, destes últimos cinco anos.

O próximo gráfico apresenta a relação anual entre o número de investigações aber-
tas e concluídas, demonstrando o esforço desenvolvido no sentido de diminuir o
backlog existente
A partir do ano de 2018 tem-se verificado um aumento do número de processos de
investigação concluídos relativamente ao número de processos abertos o que tem
permitido diminuir o backlog existente, a uma média de 40% / ano, nestes últimos
três anos.

PROPOSTAS DE AÇÃO PREVENTIVA

As Recomendações de Segurança obedecem aos princípios e requisitos definidos
nas normas e práticas recomendadas pela ICAO, bem como aos procedimentos
previstos no Regulamento (UE) n.º 996/2010 e não constituem, em caso algum,
atribuição de culpa ou responsabilidades relativamente ao acidente ou incidente na
aviação civil, nem aos, direta ou indiretamente, envolvidos.
As Propostas de Ação Preventiva (PAP) sugerem a adoção de medidas de segurança
que se consideram apropriadas, não exigindo uma resposta formal, tendo unica-
mente como finalidade incentivar as partes interessadas (stakeholders) a tomarem
medidas pró-ativas nas lacunas de segurança identificadas nos respetivos processos
de investigação.

The Safety Recommendations comply with the principles and requirements set out
in ICAO’s standards and practices, as well as the procedures set out in Regulation
(EU) No 996/2010 and do not under any circumstances constitute attribution of
guilt or liability to the accident or incident in civil aviation, or to those directly or
indirectly involved.
The Proposals for Preventive Action (PAP) suggests the adoption of safety
measures that are considered appropriate, not requiring a formal response, with
the sole purpose of encouraging stakeholders to take proactive action in the safety
gaps identified in their investigation processes.

Das 112 recomendações de segurança formuladas entre 2016 e 2020, 36 encon-
tram-se abertas, 34 das quais se reportam ao período entre 2016 e 2018 e deste
computo, 41% pertencem a uma única entidade.
Salienta-se que as 14 recomendações de segurança emitidas em 2019 se encon-
tram fechadas sendo que apenas duas não foram aceites pela entidade recomen-
dada.
As duas recomendações de segurança abertas, formuladas no ano de 2020, encon-
tram-se em fase de implementação devendo ficar encerradas durante o ano de
2021.
Of the 112 safety recommendations formulated between 2016 and 2020, 36 of
them are open, 34 of which refer to the period between 2016 and 2018 and in this
group, 41% belong to the same entity.
It should be noted that the 14 safety recommendations issued in 2019 are closed
and only two have not been accepted by the recommended entity.
The two open safety recommendations, formulated in 2020, are in the implemen-
tation phase and should be closed through 2021.

O ano de 2017 registou o maior número de fatalidades correspondendo a 2 fatali-
dades por acidente. Já o ano de 2019 regista o mesmo número de lesões do ano de

Nos últimos cinco anos a Aviação Geral (AG) registou o maior número de fatalida-
des (45%) em linha com o número de acidentes nesta atividade aérea. O Trabalho
Aéreo (TA) representa 32% e a aviação ultraleve (ULM) 23% das fatalidades regis-
tadas nos últimos cinco anos em acidentes na aviação civil.
A tabela seguinte apresenta, de forma sucinta, o número de acidentes e fatalidades
por tipo de aeronave.

ties (45%) in line with the number of accidents in this aerial activity. Air Work (TA)

Relativamente à distribuição dos incidentes investigados, por fase de voo, 57%
registaram-se na Aproximação/Aterragem (AP/AT). Na rolagem/Placa, registaram-
se 19%, na descolagem 14% e em manobra 10%.
De realçar que, no ano de 2020, não se verificou a ocorrência de qualquer incidente
grave ou incidente passível de abertura de processo de investigação técnica formal.

Regarding the distribution of the incidents investigated, by flight phase, 57% were
registered in the Approach/Landing (AP/AT) phase. The figure for taxying/ramp,
was 19%, take-off 14% and manoeuvring 10%.

incident resulting in a formal investigation process.

Pela análise do gráfico G20 verificamos que, nos últimos cinco anos, a perda de
controlo em voo (LOC-I) é o responsável pelo maior número de acidentes com
fatalidades (59%) e ainda responsável por 22% dos acidentes sem fatalidades.
38% dos incidentes foram causados por contacto anormal com a pista / aterragem
dura (ARC).

By analysing graph G20, we verified that, in the last five years, loss of in-flight con-
trol (LOC-I) is responsible for the highest number of fatality accidents (59%). In
accidents without fatalities the same type of event (LOC-I) is responsible for 22%.
38% of incidents were caused by abnormal contact with the runway/hard landing
(ARC).

O maior número de incidentes ocorreu no transporte aéreo comercial, seguido no
combate a incêndios.
