A combinação de nivolumabe e ipilimumabe é mais eficaz do que o uso das drogas isoladas, particularmente no grupo com expressão de PD-L1 < 1%. Em casos de expressão de PD-L1 > ou =1%, o uso isolado do inibidor de PD-1 provavelmente é adequado. A combinação apresenta aumento significativo de toxicidade e custo;
Ipilimumabe isolado não pode ser mais considerado tratamento imunoterápico de primeira linha padrão em melanoma avançado.
Estimados nos EUA 225000 novos casos/ano, com 160000 mortes. No Brasil, estimados 27000 casos em 2014;
Diagnóstico frequentemente tardio, com 2/3 dos casos em estádios III/IV;
Mortalidade por CBNPC é superior à mortalidade combinada de mama, pâncreas, próstata e colorretal combinados.

Nivolumabe superior a docetaxel em tto. de segunda linha de CBNPC epidermóide (017) e não-epidermóide (057) em todos desfechos (TR, SLP e SG);
Toxicidade significativamente maior com a QT em comparação a imunoterapia;
A expressão de PD-L1 foi preditiva de benefício da imunoterapia no estudo 057, mas não no 017;

Resultado dos estudos randomizados em andamento;
Intervalo de administração (cada 3 semanas Pembro x 2 semanas Nivo);
Custo;
Presença de biomarcadores preditivos, possibilitando melhor seleção dos pacientes;
Experiência do médico.
Sem estudos comparativos. Sem claras diferenças
em eficácia e toxicidade;
A imunoterapia com inibição dos checkpoints do sistema imune é uma terapia inovadora e revolucionária, por sua capacidade de induzir respostas muito prolongadas (casos com 10 anos de acompanhamento em remissão) em pacientes com múltiplos tumores avançados e refratários a outras terapêuticas;
Muitas questões ainda a serem esclarecidas: (biomarcadores, inib. PD-1 X PD-L1, monoterapia ou combinação...).
