Com vocês o nosso número 8. Estamos, como
sempre, muito felizes em entregar a vocês este
trabalho. Felizes porque estamos conseguindo,
seguindo em frente na batalha neste momento tão
complicado do país. Felizes porque sabemos que
estamos entregando uma boa revista. Felizes porque
estamos merecendo a confiança de tantos colegas tão
brilhantes e ocupados, que nos permitem publicar seus
trabalhos. Felizes porque a Revista é mais uma forma
da CIPERJ cumprir sua função de educação continuada
e apoio à especialidade no Rio de Janeiro e no país.
Aproveitamos este editorial para agradecer
(muito!) aos nossos revisores. Este é um trabalho
pesado e invisível, mas fundamental.
Só para mostrar para vocês como a coisa é feita:
cada trabalho que chega a nós é imediatamente
destituído de qualquer identificação, para eliminar
qualquer viés pessoal no exame do artigo e tornar as
avaliações o mais justas possível. Geramos assim uma
versão que retira nomes de autores e instituições e
qualquer referência que possa identificar o grupo
responsável pela autoria do artigo. A seguir esta versão
é encaminhada para dois revisores editoriais, que têm
um prazo médio de 3 semanas para fazer uma revisão
extensa no trabalho. Esta revisão inclui checar a clareza
do texto, a formatação de acordo com as regras da

revisado com as sugestões é encaminhado aos autores
para, caso aceitem, fazer as modificações.
Ao ser devolvido, o trabalho é novamente
revisado, pelo editor-chefe ou, em caso de grandes
modificações, novamente pelos revisores. Quando
acontece um “empate” (um revisor aceita e outro recusa o
trabalho) um terceiro revisor examina o trabalho.
Como vocês podem ver um trabalho enorme, feito
por pessoas ocupadíss imas e ext remamente
capacitadas, que doam seu tempo pela nossa sociedade,
pela nossa profissão e pelo crescimento de todos, já que
as crít icas editoriais sempre engrandecem os
manuscritos e servem de aperfeiçoamento para os
autores.
Recentemente, instituímos um certificado de
revisor, que será emitido em paralelo com os certificados
de aceitação dos trabalhos para finalidades curriculares.
Podem ser usados no currículo Lattes (atuação como
revisor de periódicos), citando o ISSN da Revista CIPERJ
(2318.9509) e para finalidades de comprovação da
aceitação de trabalho científico (importante para pós-
graduações e trabalhos de conclusão de curso).
Aproveitando o momento, gostaria de comentar a
regra que usamos, já que aceitamos um número limitado
de autores para casos clínicos. Seguimos uma diretriz
sugerida pela Associação Internacional de Editores
Médicos, que por sua vez se baseiam em diretrizes éticas
mundiais (http://www.wame.org/about/recommendations-on-publication-
ethics-policie). Outro comentário, talvez tardio, diz respeito à
nossa opção por publicar textos em inglês. Esta opção foi
feita para permitir o acesso de mais pessoas aos
trabalhos, popularizando eventualmente alguns artigos
além das nossas fronteiras, através da grande rede. Não
se trata, de forma alguma, de minorar a importância do
nosso belíssimo idioma.
Neste número, bastante variado, debatemos
segurança do paciente, assunto importante e pouco
discutido. Outro artigo aborda o tratamento endoscópico
das ureteroceles, com resultados excelentes. O que nos
faz pensar nas dificuldades que muitos têm para o acesso
ao instrumental, na rede pública ou na privada, onde a
relação custo-demanda faz questionar a sabedoria de
adquirir uma aparelhagem tão custosa e frágil para uso
individual. Outros trabalhos abordam a reconstrução
estética nos teratomas sacrococcígeos (um aspecto que
é rarissimamente abordado tanto em livros-texto quanto
em artigos publicados), tratamento perinatal da síndrome
CHAOS e opções terapêuticas nos hemangiomas de
parótida. O último trabalho discute os problemas
logísticos que todos nós reconhecemos para o
tratamento das criptorquias no Rio de Janeiro. E ainda
temos a sessão classificações (sobre os super-
contemporâneos critérios de Roma para distúrbios
funcionais do trato digestivo) e nossa coluna de
economia.
Espero que vocês gostem. E que discutam,
polemizem, ampliem a discussão. Em seus serviços, com
cartas para a Revista, no intervalo do café, esperando o
anestesista, na volta para casa com o colega de carona.

EFICÁCIA DA PUNÇÃO ENDOSCÓPICA DE URETEROCELE NA INFÂNCIA

RESUMO
Objetivo: Avaliar a eficácia e complicações da
punção endoscópica de ureterocele em crianças
acompanhadas no Hospi ta l In fant i l Pequeno
Príncipe, Curitiba - PR em um período de 4 anos.
Método: Estudo retrospect ivo, t ransversal e
observacional através da análise de prontuários.
Resultados: Foram avaliados 38 pacientes, 22 do
sexo feminino (57,8%) e 16 do sexo masculino
(42,2%) com média de idade ao procedimento de
1,12 anos. Os sinais e sintomas mais relatados
foram: infecção do trato urinário 16 (42,1%),
refluxo vesicoureteral 10 (26,4%) e prolapso 3
( 7 , 8 % ) . N o v e p a c i e n t e s ( 2 3 , 7 % ) e r a m
assintomáticos. Não houve relato de hipertensão
arterial. O sistema era duplo em 29 crianças
(76,3%) e o lado mais comum foi o esquerdo 18
(47,4%). O tempo médio de internação foi de 1,2
dias. Nenhuma criança apresentou complicação
pós-operatória imediata e somente 6 apresentaram
i n f e c ç ã o u r i n á r i a d o c u m e n t a d a a p ó s o
procedimento (15,7%). A ef icácia da punção
endoscóp i ca como p roced imen to ún i co em
crianças com ureterocele foi de 89,5%, sem
compl icações maiores . Somente 1 pac iente
apresentou indicação de re implante ureteral
devido a refluxo detectado após procedimento
e n d o s c ó p i c o e o u t r o s 2 n e c e s s i t a r a m d e
reimplante devido à persistência de refluxo.
Conclusão: A punção endoscópica de ureterocele
possui altas taxas de sucesso e, juntamente com o
reconhecimento de que o refluxo vesicoureteral
associado com ureteroceles pode ser conduzido de
forma eficaz conservadoramente, alterou nos
últimos anos o paradigma do tratamento nestas
crianças.

ABSTRACTS
O b j e c t i v e : To e v a l u a t e t h e e f f i c a c y a n d
complications of endoscopic ureterocele puncture
in children treated at Hospital Infantil Pequeno
Príncipe, Curitiba-PR in a 4 years period.
Method: Ret rospect ive , c ross-sect iona l and
observat ional study through medical records
analysis.
Results: A total of 38 patients were evaluated: 22
females (57.8%) and 16 males (42.2%) with a mean
age of 1.12 years. The most commonly reported
signs and symptoms were: urinary tract infection 16
(42.1%), vesicoureteral reflux 10 (26.3%) and
prolapse 3 (7.8%). Nine patients (23,7%) were
asymptomatic. There was no report of arterial
hypertension. The system was duplicated in 29
children (76.3%), most commonly at the left side 18
(47.4%). They had an average hospitalization of
1 .2 days. No ch i ld ren presented immedia te
postoperative complications and only 6 patients
had documented urinary tract infection after the
procedure (15.7%). The efficacy of endoscopic
puncture as a single procedure in children with
u r e t e r o c e l e w a s 8 9 . 5 % , w i t h n o m a j o r
complications. Only 1 patient was submitted to a
ureteral reimplantation due to reflux detected after
e n d o s c o p i c p r o c e d u r e . A n o t h e r 2 r e q u i r e d
reimplantation due to persistent reflux.
Conclusion: Endoscopic puncture of ureterocele
has high success rates and, together with the
recognition that vesicoureteral reflux associated
with ureteroceles can be effectively managed
conservatively, has altered in recent years the
paradigm of treatment in these children.

INTRODUÇÃO
A ureterocele é uma d i la tação cís t ica
congênita da porção submucosa do ureter na
região do óstio distal. Pode ser classificada como
ectópica ou intravesical além de estar associada a
um sistema renal único ou duplicado. A incidência é
maior no sexo feminino, numa proporção de 7:3 e
em caucasianos. Ocorre em 1: 4.000 crianças.
Geralmente o lado esquerdo é mais afetado e 10%
dos casos ocorrem bilateralmente 1,2.
O p a c i e n t e p o d e s e a p r e s e n t a r
assintomático ou sintomático, sendo a infecção
urinária uma das principais manifestações. O
quadro pode evoluir com septicemia.
Outras sintomatologias decorrentes seriam
a l t e r a ç õ e s g a s t r o i n t e s t i n a i s , r e t a r d o n o
desenvolv imento da cr iança, dor em f lanco,
hematúria, prolapso e obstrução miccional 3.
A suspeita clínica pode ser confirmada por
ultrassonografia através da visualização de uma
lesão c ís t i ca i n t raves ica l , u re t roc i s tog ra f i a
miccional (UCM) ou urografia excretora com uma
imagem negativa na parede posterior da bexiga. A
UCM também é importante para avaliação da
presença de refluxo vesicoureteral, presente em
50% dos casos na unidade inferior do sistema
duplicado e em 25% no ureter contralateral. O
exame de cintilografia renal com radioisótopos
(DMSA) deve ser realizado para auxiliar na decisão
de preservação ou não do segmento drenado pela
ureterocele 4,5. Apenas 10 a 25% dos casos
associados a ureteroceles ectópicas apresentam
função adequada. A tomografia computadorizada e
ressonância magnética podem ser utilizadas em
situações de dúvida diagnóstica ou em casos de
rins com sistema coletor de aparência bizarra e nas
duplicações com ambas as pelves e ureteres
dilatados.
O ob je t i vo em t ra ta r a u re te roce le é
preservar a função renal, prevenir infecções
causadas por ref luxo e obstrução e também
promover a manutenção da continência urinária.
Entretanto, não há um método cirúrgico que
c o n t e m p l e s u f i c i e n t e m e n t e t o d a s e s t a s
expectativas 6.
Muitos estudos realizados no decorrer dos
últ imos vinte anos favoreceram a abordagem
e n d o s c ó p i c a p a r a s i s t e m a s i n t r a v e s i c a i s ,
ectópicos, sistemas único e duplo.
A maior parte da literatura tem mostrado
esta eficácia, principalmente em crianças 7.
O presente artigo tem o objetivo de avaliar a
eficácia e complicações da punção endoscópica de
u re te roce le em c r ianças acompanhadas no
Hospital Infantil Pequeno Príncipe, Curitiba -PR em
um período de 4 anos.
MÉTODOS
E s t u d o r e t r o s p e c t i v o , t r a n s v e r s a l e
observacional através da análise de prontuários
entre junho 2012 a agosto de 2016. Os dados foram
agrupados e analisados de forma descritiva no
p r o g r a m a M i c r o s o f t E x c e l ® . O p r o j e t o f o i
previamente aprovado pelo Comitê de Ética da
Instituição com o número 55805916.7.0000.0097.
RESULTADOS
Foram avaliadas 38 crianças, 22 do sexo
feminino (57,8%) e 16 do sexo masculino (42,2%)
com média de idade ao procedimento de 1,12 anos,
idade mínima de 2 meses e máximo de 15,9 anos.
Os sinais e sintomas mais relatados podem ser
vistos na Tabela 1. Nove pacientes (23,7%) eram
assintomáticos, desses quatro com diagnóstico no
período pré-natal. Não houve relato de hipertensão
arterial.

Nove c r ianças apresen tavam s is tema
simples 23,7% e 29, 76,3% sistema duplicado. O
lado mais comum foi o esquerdo, 18 (47,4%),
seguido pelo direito 15 (39,5%) e bilateral 5
(13,1%). Vinte e cinco ureteroceles (65,8%) eram
ectópicas e 13 (34,2%) intravesicais.
O tempo médio de in ternação para o
p roced imento endoscóp ico fo i de 1 ,2 d ias .
Nenhuma criança apresentou complicação pós-
opera tór ia imed ia ta e somente 6 pac ien tes
apresentaram infecção urinária documentada após
o procedimento no seguimento ambulator ia l
( 1 5 , 7 % ) . E s t e s p a c i e n t e s t a m b é m t i n h a m
apresentado infecção urinária previamente ao
procedimento.
Somente 1 criança necessitou de uma nova
punção da ureterocele 6 meses após o primeiro
p r o c e d i m e n t o . F o r a m f e i t o s 3 r e i m p l a n t e s
u re te ra i s , do i s po r pe rs i s tênc ia de re f l uxo
vesicoureteral e um com novo refluxo após punção
para a unidade superior drenada pela ureterocele,
em média 20 meses após o primeiro procedimento.
Um paciente necessitou de pielostomia devido a
infecção ur inár ia de repet ição, mesmo após
procedimento endoscópico, 9 meses após o
procedimento. A evolução do refluxo vesicoureteral
nesses pacientes pode ser vista na tabela 2.
Portanto, a eficácia da punção endoscópica
como proced imento ún ico em cr ianças com
ureterocele foi de 89,5%, sem complicações
ma io res . Somente um pac ien te apresen tou
indicação de reimplante ureteral devido a refluxo
detectado após procedimento endoscópico (Tabela
3).
DISCUSSÃO
As vantagens da punção endoscópica no
tratamento da ureterocele são a simplicidade, o
fa to de ser um proced imento min imamente
invasivo, que requer anestesia breve e pouco
complexa e geralmente pode ser realizado de
forma ambulatorial 8. A média de internação dos
pacientes neste estudo foi de 1,2 dias.
De acordo com Jesus et al 9, que estudaram
a evolução clínica do refluxo vesicoureteral após
punção endoscópica de ureterocele em pacientes
pediátricos com sistema duplo, o refluxo não está
d i r e t a m e n t e r e l a c i o n a d o c o m a p u n ç ã o
endoscópica pois ele pode ocorrer nos pólos
superior ou inferior ipsilateral ou nos segmentos
renais contralaterais. Quarenta por cento dos
pac ien tes em pós -ope ra tó r i o t i ve ram novo
a p a r e c i m e n t o d e r e f l u x o v e s i c o u r e t e r a l ,
pr incipalmente ipsi lateral no pólo infer ior ou
contralateral, porém a resolução espontânea ou
r e d u ç ã o d o g r a u o c o r r e u e m u m n ú m e r o
s ign i f i can te de c r i anças , 72%. Os au to res
concluem que a punção endoscópica é uma boa
o p ç ã o d e t r a t a m e n t o i n i c i a l e m p a c i e n t e s
pediátricos com ureterocele em sistema duplo.

procedimento endoscópico, com somente um caso
novo para a unidade superior após a punção,
15,8% apresentaram resolução espontânea 9.
Adorisio et al 10 também realizaram estudo
do tratamento endoscópico de ureterocele em 46
pacientes pediátricos com ureterocele ectópica em
s i s t e m a d u p l o . E m 9 3 % d o s p a c i e n t e s , o
tratamento garantiu descompressão. Apenas três
pacientes necessitaram de nova cirurgia para
tratamento de ureterocele persistente. Após os
procedimentos, nenhum paciente apresentou
deter ioração da função renal . Em 30% dos
p a c i e n t e s ( 1 4 c r i a n ç a s ) h o u v e r e f l u x o
vesicoureteral, com resolução espontânea em 10
crianças. Os autores concluem que a punção
endoscópica primária da ureterocele é simples,
segura e eficaz a longo prazo 10.
Chertin et al 11 concluíram que a punção
endoscópica primária de ureterocele a longo prazo
é eficaz e segura, evitando uma reconstrução
completa a partir de seu estudo com 52 pacientes.
Houve descompressão completa da ureterocele
em 48 casos após a primeira punção endoscópica.
Quatro necessitaram de uma segunda punção da
ureterocele. Nove crianças foram submetidas a
nefrectomia por rins não funcionantes. Cinco
tiveram refluxo vesicoureteral nas porções do pólo
superior após a punção 11. Novamente em outro
estudo, Chertin et al 12 também concluíram que o
tratamento endoscópico do refluxo vesicoureteral
associado com ureterocele é uma técnica segura,
simples e eficaz, evitando necessidade de cirurgia
aberta na maioria dos doentes após punção
endoscópica. Eles anal isaram 109 pacientes
pediátricos. Em 85 casos de refluxo vesicoureteral
pós punção, 42% obtiveram resolução espontânea

A punção endoscópica de ureteroceles
possu i a l t as t axas de sucesso . Embora a
reconstrução total de frações renais associadas
com ureterocele possa ser atraente, pois é
p o s s í v e l c o n s e g u i r u m t r a t o u r i n á r i o
a p a r e n t e m e n t e n o r m a l c o m u m ú n i c o
p r o c e d i m e n t o r e a l i z a d o n a i n f â n c i a , u m a
abordagem mais individualizada que se baseia em
t ra tamentos c i rú rg i cos menos agress ivos e
tratamento não cirúrgico ao longo do tempo pode
conseguir o mesmo resultado funcional 13.
CONCLUSÃO
A punção endoscópica de ureterocele possui
altas taxas de sucesso, e juntamente com o
reconhecimento de que o refluxo vesicoureteral
associado com ureteroceles pode ser conduzido de
f o rma e f i caz conse rvado ramen te , mudou o
paradigma para uma abordagem individualizada
com maior ênfase colocada no tratamento não-
cirúrgico ou técnicas cirúrgicas menos agressivas.

ABSTRACTS:
Sacrococcygeal teratomas (SCT) post-resection
reconstruction is not extensively described. Research
focuses mainly on malignant potential, post-operative
sequelae and control of perioperative risks. We describe
a V-Y method for skin closure after SCT resection, aiming
to provide a more natural buttocks contour and to hide
scars inside underwear or bathing suits.

INTRODUCTION:
Sacrococcygeal teratomas (SCT) are huge and
hypervascularized tumors, most frequently exophytic,
presenting in neonates. Research on SCT is mostly
phocused in their malignant potential, post-operative
sequelae and control of perioperative risks, especially
exsanguination. There are few data about buttocks
contour reconstruction and cosmetic results after SCT
resection, but unsightly scars and unnatural buttocks
contour may cause serious problems for those patients,
mostly females, causing social embarrassment, restriction
of usage of bathing suits and sexual avoiding behavior in
adolescents.
Our objective is to propose a V-Y method for skin
closure after SCT teratoma resection, aiming to provide a
more natural buttocks contour and to hide scars inside
underwear or bathing suits.
CASE REPORT:
A 3 days-old full-term female was operated on for
resection of a sacrococcygeal teratoma (SCT). The tumor
was first diagnosed in a third trimester obstetric
ultrasound. The neonate presented a 12 cm diameter
irregular, solid-cystic SCT (figure 1.1). The tumor was
classified as an Altman 2 SCT after an abdominopelvic CT.
A prone jackknife position with pelvic elevation was
chosen and an extensive Chevron incision was used for
the resection. A 680g tumor was excised successfully
together with a complete coccygectomy. The continence
muscles were sutured in the sagittal median plane. An
extensive inferior cutaneous flap was then evidenced.
This flap was trimmed as a omega-shaped inferiorly based
flap (figure 2). This flap was fixed to the deep muscular
plane in the median line, in order to recreate the
intergluteal sulcus. The lateral extremities of the flap were
then sutured to the superior cutaneous flap in a caudal-
cranial direction, pulling the scar towards the median live
and creating two symmetric parallel scars, 2 cm lateral to
the intergluteal sulcus. Finally, the “dog ear” that was

created in the median segment of the superior side of the
Chevron incision was trimmed and sutured vertically in
the median plane (figure 2). The final result was an
inverted “Y” scar (figure 1.2). After 3 months the patient
presents normal serum levels of alpha-fetoprotein and an
inconspicuous scar, easily hidden by underwear (figure
3).
DISCUSSION:
The cosmetic results after SCT resections are
rarely discussed. Unsightly scars are the rule, sometimes
associated to abnormal buttocks contour. Long-term
esthetic problems are, indeed, the most common
complication after SCT resection [1]. This may cause
serious social restrictions and eventual psychological
problems to young females that survive the disease,
especially during sexual intimacy and in hot climates,
where the usage of bathing suits is frequent in social life.
In a retrospective cohort all interviewed patients reported
poor cosmetic results and 8/9 were either considering or
had already been submitted to corrective plastic surgery [1].
The direct closure of the classical Chevron
incisions with redundant skin frequently creates inverted
“C” scars that extend to the lateral buttocks and may
cross the infragluteal creases to the proximal thighs. A
closing technique that is able to hide the scars centrally
may be esthetically advantageous. We also believe that
the central fixation of the inferior cutaneous flap is useful
to create a normal intergluteal sulcus, preventing a “flat”
buttock final appearance.
Fishman et al [2] proposed the usage of 3
cutaneous translation flaps in order to attain similar results
to our simpler proposal. Their method, however, differs in
the final appearance of the scar that results in a bit more
extensive “π” than ours inverted “Y” final scar. Post-
resection lateral contour in their cases also seems flatter
than in our patient. That may be due to either to the tension
forces in the superior horizontal bar on the “π” scar in their
technique or to the absence of a deep skin fixation to the
median line, to reinforce the intergluteal sulcus.
Other authors proposed a sagittal cutaneous
incision, followed by centrally excising an elliptical
segment of redundant skin as needed and a vertical
median skin suture after excising the tumor [3]. In our
opinion, this approach has two major problems: (1) a
sagittal incision is more restrictive and may hamper the
resection of the tumor and (2) the median vertical closure
creates an extensive median scar, extending higher over
the spine than the vertical segment of our inverted Y
suture.
To conclude, we believe that our proposal is a
simple and feasible method for skin closure after SCT
resection, able to disguise post-operative scars and to be
effectively covered by underwear or bathing suits, also
creating a normal buttocks contour.

characterized by fetal airway obstruction. The aim
of this study was to report the case of a newborn
(NB) prenatally diagnosed with CHAOS. A female
baby was born at 36 weeks of gestation. The
mother was 33 years old without comorbidities.
D u r i n g p r e n a t a l f o l l o w - u p , m u l t i p l e f e t a l
malformations were found at routine ultrasound
(US) and the mother was submitted to magnetic
r essonance imag ing (MRI ) . She wen t i n t o
premature labor and the cesarean section had a
multiprofessional assistance. An EXIT (ex utero
intrapartum treatment) was performed: there was a
bl ind-ending trachea 1 centimeter below the
thyro id car t i lage and the rest o f the organ
consisted of a fibrous cord without lumen. The NB
died in the del ivery room. CHAOS is a rare
syndrome with high mortal i ty. I f undiagnosed
prenally, it frequently leads to fatal outcome.

INTRODUCTION

s y n d r o m e ( C H A O S ) i s a r a r e c o n d i t i o n
characterized by complete or partial obstruction of
the fetal upper airways. [1,2]
If unrecognized during the prenatal period, it
usually results in stillbirth or death shortly after
delivery. [3] Fortunately, more cases are being
recognized in utero, as there have been significant
improvements in prenatal imaging.
Before attempts of fetoscopic fetal salvage
or ex-utero intrapartum treatment (EXIT) are
considered, addit ional malformations must be
carefully studied.
The aim of this study is to report the case of a
newborn (NB) prenatally diagnosed with CHAOS.
CASE REPORT
A 33 yea r -o ld mo the r, g rav ida 1 and
gestational age of 35 weeks, was referred to our
inst i tut ion due to fetal anomal ies on rout ine
ultrasound (US) during the 18th week of gestation:
bilateral hydronephrosis, increased volume of lungs
suggestive of bilateral congenital pulmonary airway
malformation (CPAM) and right hand malformation.

The amniotic fluid index was normal.
The pat ient 's pregnancy, personal and
family history were unremarkable. There was no
history of tobacco, alcohol and illicit drug use.
US at our hospital was performed: the
estimated weight was 1400 g. The baby had large
echogenic lungs, fetal hydrops, polyhydramnios
and centrally placed heart (Fig. 1).
CHAOS was suspected and, considering
the possibi l i ty of EXIT, magnetic ressonance
i m a g i n g ( M R I ) w a s p e r f o r m e d . T h e e x a m
conf i rmed volumetr ic increase of the lungs,
polyhydramnios, diaphragmatic inversion, fetal
hydrops, as well as tracheal alterations suggestive
o f a t res ia , a l t hough the s i t e cou ld no t be
characterized (Fig. 2).
The mother was submitted to caesarian
section at 36 weeks of gestation due to premature
labor and had a multiprofessional assistance.
The baby was female and weighed 1455 g.
She didn't show any respiratory movements after
delivery and EXIT was performed: one centimeter
below the thyroid cartilage, the trachea was atresic
and the distal part consisted of a fibrous cord
without lumen, which prevented the tracheostomy
(Fig. 3).
After 3 minutes, the baby had cardiac arrest
and was declared dead. In addition to the airway
malformation, the baby had low implantation of the
ears and agenesis of the right hand.
DISCUSSION
CHAOS is a rare disorder that is defined by
a fetal abnormality that obstructs the larynx or
trachea intrinsically (laryngeal atresia being the
most common) and leads to entrapment of fluid
within the tracheobronchial tree [5,6].
In a healthy fetus, fluid secreted by the lung
is absorbed through the tracheobronchial tree. In
case of obstruction, this fluid cannot be cleared and
its accumulation results in increase of intra-tracheal
pressure, leading to enlargement of the lungs. It is
the beginning of a chain reaction: the enlarged
lungs compress the hear t and g rea t ve ins ,
centralizing the heart, which becomes small and
dysfunct ional . Decreased venous return and
dysfunctional cardiovascular system results in
ascites and hydrops. The diaphragm flattens or
inverts according to the severity of the process. [3]
The cause and incidence are not known.
Most authors suggest these malformations occur
when the upper airway fails to recanalize around
the 10th week of gestation. Genetic, vascular and
sporadic causes have also been proposed, the
latter being the most common. [3,7]
Precise prenatal assessment is essential in
counseling the family, as well as management and
therapeutic intervention planning. [2] Besides, if not
diagnosed prenatally, CHAOS will probably lead to
an unanticipated fatal outcome at delivery. [8]
Due to its low cost and wide-spread use, the
first-line prenatal diagnostic tool for CHAOS is US,
which has typical findings on evaluation: bilateral
increased and hyperechoic lungs, smal l and
cent ra l ly p laced hear t , f la t tened or inver ted
diaphragm and ascites. [4] The enlarged lungs may
compress the superior vena cava, thoracic duct and
heart, diminishing the venous return and causing
ascites, fetal hydrops and placentomegaly. [2]

pregnancy, due to the absence of lung contribution
to the total volume of amniotic fluid. It is also
p r e s e n t i n c a s e s o f F r a s e r s y n d r o m e , a n
autosomal recessive disease, in which renal
agenesis may be associated with cryptophthalmos,
abnormalities of the ears, syndactyly and laryngeal
atresia [9] Compression of the esophagus by
di lated airways may lead to polyhydramnios,
usually later in pregnancy, as the fetal swallowing
of the fluid is disrupted. [3] Therefore, the amount
of amniotic fluid is not a constant marker for the
diagnosis. Normal amniotic fluid index can also be
found in CHAOS, as described by Ulkumen et al. [10]

effect iveness in detect ing the exact level of
obstruction, a critical step in surgical planning, as
there must be adequate space for a tracheostomy
tube between the obstruction and the thoracic
inlet. [11]
The coexistence of tracheo-esophageal
fistula (TEF) with CHAOS is expected to neutralize
the pressure effect on the developing lungs and to
lead to normal-sized, making the diagnosis more
challenging. [11]
Once CHAOS is diagnosed, addi t ional
malformations must be investigated, because they
can affect the prognosis and are present in about
50% of fetuses with CHAOS. [2]
Di fferent ia l d iagnosis includes bi lateral
presentat ion of congeni ta l pulmonary a i rway
malformation (CPAM). Color Doppler may be
helpful, since fluid flow in the trachea can be
demonstrated during fetal breathing in CPAM, in
con t ras t t o CHAOS. [ 6 , 7 ] Bes ides , t r achea l
enlargement below the obstruction site is specific to
CHAOS. [5]
The syndrome should also be differentiated
f r o m e x t r i n s i c c a u s e s o f t r a c h e o l a r y n g e a l
obstruction, such as lymphatic malformations,
cervical teratoma and vascular rings. [4]
The point to take into consideration is the
necessity of a detailed evaluation of all CHAOS
suspected cases due to possibility of coexistence of
any genetic syndrome and significant implications
o f i n h e r i t a n c e f o r f u t u r e p r e g n a n c i e s . [ 2 ]
Nevertheless, in countries that allow pregnancy
termination, it is a possibility for severe cases with
mu l t i p le congen i ta l abno rma l i t i es and poo r
prognosis. [5,10,11]
Intrauterine treatment and EXIT are options
available to manage this condition. A percutaneous
fetoscopic approach can be undertaken in selected
cases, but premature rupture of membranes is a
p o s s i b l e c o m p l i c a t i o n . C o n c e r n i n g f e t a l
tracheostomy, it is uncertain what effect it would

have on lung development in fetuses with CHAOS,
particularly since chronic drainage of fetal lung
fluid has been shown to result in pulmonary
hypoplasia in sheeps. [12]
For patients who are not candidates for
in t ra -u te ro in te rvent ion , EXIT de l i very w i th
tracheostomy remains the treatment option with
the best chance for survival. The objective of the
procedure is to evaluate and secure an airway,
below the level of obstruct ion, whi le utero-
placental gas exchange is preserved. [1]
I n 1998 , De Cou e t a l . repor ted the
successful use of EXIT for CHAOS due to laryngeal
atresia. It is known, though, that cases of tracheal
agenesis or atresia cannot be salvaged, not even
by EXIT. And, even in successfull EXIT cases, long-
term survivors without brain damage or permanent
tracheostomy are very rare. [1]
These exceptional and complex techniques
must only be offered by highly exper ienced
multidisciplinary fetal surgery teams with high-
level of postnatal neonatal and pediatric surgery
care.
Considering the long-term outcomes, the
parents should be prepared for the significant long-
term morbidity associated with this condition,
inc lud ing long- te rm t racheostomy, impa i red
speech, chronic respiratory problems, and the
need for several operations. [13]
The ideal timing of reconstruction in these
patients has not been established [2], but it is
generally deferred until the child is at least 2 years
old and it usually requires a staged procedure. [13]
Even in the best case scenario, CHAOS patients
will have definite functional limitations affecting
their quality of life.
CONCLUSION
CHAOS is a rare condition and it is fatal if
u n r e c o g n i z e d d u r i n g p r e n a t a l p e r i o d .
Consequent ly, the importance of predel ivery
planning cannot be overemphasized.
Fetal surgery and EXIT are the available
modalities for CHAOS patients since the perinatal
mortality is 100% without intervention, secondary
to respiratory failure.
I n v o l v i n g a m u l t i d i s c i p l i n a r y t e a m o f
neonatologists, radiologists, obstetr icians and
anesthes io log is ts is the keys tone o f p roper
management, but management of the airway,
particularly regarding long-term reconstruction,
remains very complex and challenging.

ABSTRACT
Background / Purposes: I n a deve lop ing
country, like Brazil, health campaigns aimed to
reduce the waiting time for performing several
elective surgeries. However, this may represent
risk situations regarding surgical safety. Multiple
s u r g e r i e s i n v o l v i n g s i m i l a r d i a g n o s e s a r e
performed simultaneously, which may facilitate
the occurrence of errors. We aimed to analyze the
efficiency of using a surgical security protocol in
special situations in which a large number of
procedures occur simultaneously in a short period
of time, such as an elective surgery campaign.
M e t h o d s : A s u r g i c a l s e c u r i t y p r o t o c o l w a s
established and applied in the molds proposed by
the WHO, during a pediatric surgery campaign
occurred at the Clinics Hospital of the Botucatu
Medical School. Results: In total, 20 patients were
submitted to surgical procedures involving uni- or
bilateral inguinal hernia repair, hydrocelectomy,
umbilical hernia and circumcision. No early or
d e l a y e d s u r g i c a l c o m p l i c a t i o n s o c c u r r e d .
Conclusions: we conclude that in the context of
surgical campaigns, where the possibility of errors
is considerably greater, the use of a surgical
safety protocol proved to be both fundamental and
easy to apply.

INTRODUCTION
The idea of health campaigns in Brazil
b e g a n w i t h t h e " C a t a r a c t P r o j e c t , " r u n b y
ophthalmology services from 1987 onward (1). In a
developing country, where 80% of the population
is attended by the public health system, this
initiative aimed to reduce the waiting time for
performing several elective surgeries (2). Thus,
the Brazilian Society of Pediatric Surgery has held
"National Campaigns for Surgery in Children"
annually since 2007, in which pediatric surgeons
from all over Brazil are mobil ized to perform
e l e c t i v e s u r g e r i e s . T h o u g h c l e a r l y w e l l -
intentioned, such surgical campaigns represent
risk situations regarding surgical safety. Multiple
s u r g e r i e s i n v o l v i n g s i m i l a r d i a g n o s e s a r e
performed simultaneously, which may facilitate
the occurrence of errors (2). To prevent such errors
from ending the initiative of the campaign, which
was be held for the first time in the Clinics Hospital

of the Botucatu Medical School in 2010, a
security protocol was established and applied to
a l l pat ients f rom preoperat ive evaluat ion to
hospital discharge. Thus, we aimed to analyze
the efficiency of using a surgical security protocol
in special situations in which a large number of
procedures occur s imul taneously in a short
period of t ime, such as an elect ive surgery
campaign.
MATERIALS AND METHODS
Preoperative consultations were held for
new cl inical examinations of patients on the
waiting list for elective surgery and who showed
no comorbidities that would prevent performance
of the procedure under an outpatient regime. For
these consultations, we developed a succinct
attendance form emphasizing diagnosis, drug
al lergies and personal and family history of
coagu lopath ies . In these consu l ta t ions , we
explained the details of the procedure and fasting
was or iented, s ince the ch i ldren were only
hospitalized on the day of the surgery. A term of
f ree, in formed consent was e laborated that
clearly specified the diagnosis and the side of the
body where surgery should be performed (in
cases of inguinal hernias and hydroceles). This
term was signed by a parent or guardian at the
t i m e o f a d m i s s i o n , w h e n t h e p a t i e n t w a s
reexamined i n t he p resence o f t he same ,
confirming the exact location of the pathology,
which was immediately indicated on a graphic
record in the patient (Figure 1). A preanesthetic
consultation was held soon after, by a team of
anesthesiologists. On entering the operat ing
room (OR) , the ch i ld ' s da ta regard ing the
pathology and surgery to be performed were
checked with the parent or guardian. Once inside
the OR, all the data were rechecked a final time.
Following the procedure, patients were submitted
to cl inical observat ion in the recovery room
before being discharged. Two postoperat ive
consultations were scheduled after 7 and 30
days.
RESULTS
In total, 20 patients were submitted to
surgical procedures involving uni- or bilateral
inguinal hernia repair, hydrocelectomy, umbilical
hernia and circumcision. The patients arrived at
the hospital on the day of the procedure in the
morn ing , unde r f as t i ng . A l l t he pa ren t s o r
g u a r d i a n s s i g n e d t h e c o n s e n t f o r m a n d
accompanied the children up to entering the OR
and postsurgery, in the recovery room. The
anesthesia for al l patients was sedation with
locoregional anesthesia. The surgical procedures
were simultaneously performed in three ORs by
th ree surg ica l teams. A l l the ch i ld ren were
discharged from the recovery room straight home
following the observation period and a meal. No
early or delayed surgical complications occurred.
DISCUSSION
It is estimated that 234 million people each
year are submitted to some type of surgical
procedure (3). Of this total, up to 2 million can die
and about 7 million may present complications,
and of these, 50% are considered avoidable (2,5).

procedures and techniques should be highlighted
(5).
In order to establish a protocol that is easily
applicable to the reality of hospitals, especially
p u b l i c h o s p i t a l s a n d t h o s e i n d e v e l o p i n g
countries, the World Health Organization (WHO)
developed a checklist to be used in all surgical
p r o c e d u r e s i n a n y h o s p i t a l i n t h e w o r l d ,
regardless of their degree of complexity (4). This
p r o t o c o l w a s u s e d i n a m u l t i c e n t e r s t u d y
published in 2009, which revealed that the use of
t h e c h e c k l i s t r e d u c e d t h e r a t e o f s e v e r e
complications from 11 to 7% and the mortality rate
from 1% to 0.8% (5). Since then, this checklist has
been implemented in most hospitals in Brazil,
including ours. Caution regarding the safety of
surgery and careful evaluation of the pre-, intra-
and postoperat ive per iods are essent ia l and
should be performed at all stages of the procedure

by the surgeon (6-8).
In Brazil, where the public health system is
responsible for the vast majority of cases, waiting
lists for elective surgeries can extend for years.

pathologies (9-11).
Spec ia l care shou ld be taken dur ing
surgical campaigns. This is because several
operations with similar diagnoses are performed
simultaneously, faci l i tat ing the occurrence of
errors, such as switching the side of the surgery,
forgetting one side in the case of a bilateral
inguinal hernia and even mixing up patients (2).
T h e s u r g e o n m u s t c o n f i r m t h e d i a g n o s i s
previously and not perform the exam only in the
OR, which would make the procedure vulnerable
to problems. The preoperative evaluation should
include knowledge of personal medical history, a
complete physical examination, clarification of
medications being administered and possible
allergies (4). As with any surgical procedure,
during campaigns, it is essential that attention is
paid to the proper completion of informed consent
by those responsible and that this is attached to
the patient's medical records. It is also necessary
to explain the anesthetic and surgical techniques
that will be performed. Such measures are aimed
at patient safety and provide legal support for
medical action in any surgical situation (12).
The first experience of running a pediatric
surgery campaign at our service established the
use of a surgical safety protocol. Use of this
protocol and all the cautions described proved to
be efficient in a risk situation. No early or delayed
su rg i ca l comp l i ca t ions occur red . Thus , we
c o n c l u d e t h a t i n t h e c o n t e x t o f s u r g i c a l
campaigns, where the possibil ity of errors is
considerably greater, the use of a surgical safety
protocol proved to be both fundamental and easy
to apply.

ABSTRACTS
INTRODUCTION: Cryptorchism should be treated
between 6-18 months. Late diagnosis, referral to
specialists and unavailability of surgery may delay
treatment. Our objective is to analyze the frequency of
late orchidopexies and to verify it´s causes.
METHODS: Parents of children waiting for orchidopexies
in our institutions were interviewed between June/July
2014.
RESULTS: 31/52 parents answered to the questionnaire.
Nineteen (61.3%) cryptic testes were palpable and 20
(64.5%) were unilateral. The mothers ignored their
newborns´ problem in 21 children (67.7%). Only ¼ of the
mothers were counselled by neonatologists. In 35.6% the
diagnosis was suspected by the mother or the patient.
Three children were incidentally diagnosed by Pediatric
Surgeons. After initial consultation 26 patients were sent
to a specialist for treatment, but five were counselled to
wait till ≥ 1 year of age. The first consultation about
cryptorchism was at 1.4 year of age (median). Delay for
consultation with a Pediatric Surgeon/Urologist after
referral was 0.6 years (median). Total delay time between
primary consultation and surgery was 2.1 years (median).
CONCLUSION: Late orchidopexies in Brazilian
Universal Health System are due to late diagnosis, long
waiting times for consultation with experts and for
surgery. This may be addressed with better protocols
about infantile cryptic testicles and the need of systematic
genital examination in well-children consultations for
Neonatologists and Pediatricians. Early availability of
Pediatric Surgeon/Urologist consultation and a better offer
of operating rooms for those patients are essential. Also,
we would suggest prioritization of bilateral cases,
considering their worst fertility prognosis.

RESUMO
INTRODUÇÃO: A criptorquia deve ser tratada entre 6-18
meses de idade. Diagnóstico ou referência tardia ao
especialista e indisponibilidade de cirurgia podem
retardar o tratamento. Nosso objetivo é analisar a
frequência é as causas das orquidopexias tardias.
MÉTODOS: Os pais das crianças listadas em fila de
espera para orquidopexias foram entrevistados em
Junho/Julho 2014.
RESULTADOS: 31/52 pais responderam ao questionário.
Dezenove (61.3%) testículos crípticos eram palpáveis e
20 (64.5%) unilaterais. As mães ignoravam o problema de
seus recém-nascidos em 21 casos (67.7%). Apenas ¼
das mães foram aconselhadas por neonatologistas. Em
35.6% o diagnóstico foi suspeitado pela mãe ou pela
própria criança. Três crianças tiveram diagnóstico
incidental por Cirurgiões Pediátricos. Após a consulta
inicial 26 pacientes foram encaminhados a um
especialista para tratamento, mas 5 foram aconselhados
a esperar até ≥ 1 ano de idade. A primeira consulta para
tratar a criptorquia foi com 1,4 anos de idade (mediana). O
retardo até a consulta com um Cirurgião ou Urologista
Pediátrico depois da referência foi de 0,6 anos (mediana).
O tempo decorrido entre a consulta inicial e a cirurgia foi
de 2.1 anos (mediana).
CONCLUSÃO: Orquidopexias tardias no SUS são
devidas a diagnostico tardio, longos tempos de espera
para consulta com especialistas e para a cirurgia. Isto
pode ser corrigido com melhores protocolos para o
tratamento de criptorquidia em lactentes e exame genital
sistemático em consultas pediátricas de atenção primária.
A disponibilidade rápida de consultas com especialistas e
uma maior oferta de salas de cirurgia para estes pacientes
é essencial. Sugerimos também a priorização de casos
bilaterais, considerando o prognóstico de fertilidade pior

INTRODUCTION:
Cryptorchidism is a very common condition,
affecting 1-3% of children [1]. Classically only congenital
(CC) and iatrogenic forms of the disease were recognized.
A new variety, ascending testis (AT), has been recently
recognized [2]. In this specific situation a previously scrotal
testis ascends to the inguinal region as the child grows,
becoming cryptic. AT differ from retractile testes, as the
later ascend spontaneously to the inguinal region but can
be easily and painlessly brought to the scrotum without
tension, staying there for some time if environmental
conditions are favorable.
CC should be treated between 6 and 18 months of
age. Postponement of the treatment is related to worse
results, as histological modifications in the cryptic testis
have been demonstrated from the second semester of life,
presumably deteriorating future fertility and leading to
atrophy of the organ [3-4].
Late surgery for cryptorchidism is frequent. The
reasons may vary between late diagnosis, late referral,
unavailability of surgery and high rates of AT in the
population. Our objective herein is to investigate the
causes of orchidopexy delay in Brazilian children
presenting with cryptorchidism.
Our primary aims were (1) to verify the timing of
acknowledgement of cryptorchidism by the families, (2) to
attest the intervals between the perception of the problem
by the families, first medical consultation, specialist
consultation and surgery and (3) to establish the causes
of delaying orchidopexies in our public health system.
METHODS:
This research is a transversal descriptive study.
All children assigned in waiting lists for orchidopexy in the
Pediatric Surgery Department of Hospital Federal dos
Servidores do Estado and with one of the authors (LEJ) in
Hospital Universitário Antonio Pedro were included in our
study cohort, without exceptions (inclusion criteria:
children between 0-< 18 years-old signed in a waiting list
for orchidopexy in the participating institutions). They
were interviewed by two medical students (IBA and
RPM). The interview was made by a phone call, using a
non-standardized questionnaire with objective questions
developed by the authors (figure 1), in the months of June
and July 2014. The waiting times for surgery were
censored at the day of the research interview, for the
purposes of this project. Clinical data were obtaining by
reviewing the patients´ charts. Patients were censored at
the age they were at surgery if found to have been
operated previously at another health institution at
moment of the researchers phone call (n=3).
If the parent was unavailable after three trials in
different days the case was excluded from the research
as unreachable. All parents were informed about the
objectives of the research and the questionnaire was
applied only after their oral informed consent. Parents
that refused or declared themselves unable to answer to
the questionnaire were also excluded (exclusion criteria:
unreachable parents after three trials, inability or refusal
to answer to the questionnaire).
Testes were defined as nonpalpable by the
surgeons after physical examination in clinics, as attested

in the children´s charts.
Statistical analysis was restricted to descriptive
calculations (prevalence, distribution, calculation of
measures of central tendency and dispersion) by the
nature of our research (descriptive transversal).
RESULTS:
Fifty-three children were listed for orchidopexy in
the two waiting lists. Twenty-one parents (40.4%) were
unreachable, unable or refused to answer to the
questionnaire and were excluded from the research (ages
1-13 yo, mean 6 yo, median 5.5 yo, SD 3.4). Thirty-two
parents responded to the questionnaire. Twenty (64,5%)
children presented unilateral cryptorchidism. Nineteen
(61,3%) cryptic testicles were described as palpable by
the surgeon.
Only two children had their testis described as
topic as neonates: those two are the only cases classified
as AT in our cohort. All children were delivered in hospitals
and had expert perinatal Pediatric assistance.
T h e f i r s t m e d i c a l c o n s u l t a t i o n a b o u t
cryptorchidism was the mother´s initiative in 8 (25.8%)
children. The child asked for treatment in 3 (9.7%) cases.
Three other cases (9.7%) were incidentally diagnosed by
a Pediatric Surgeon/Urologist (PSU) consulted for another
disease. The other children (n=18, 56.3%) were either
diagnosed by Pediatricians or Primary Practitioners (PP)
in routine consultations or purposefully followed up after
the neonatologist informed the mother about the child´s
condition.
After exclusion of the three cases that were
incidentally diagnosed by PSU, 22 children (75.9%) were
sent to a PSU for a consultation immediately after
diagnosis by the Pediatrician/PP. One child was sent to an
Urologist for consultation (3.4%). Six children (20.7%)
were advised to consult an Endocrinologist (n=1, a case
of bilaterally nonpalpable testes), wait till 1 year-old for
specialized consultation (n=2) or wait till > 1 year-old for a
PSU´ evaluation (n=3).
The two cases of AT were excluded from further
analysis. Children´s ages varied between 0.7 and 12 yo
(mean 8.2, median 3.5, SD 3). Age distribution is
described in figure 1. Nineteen (63.3%) mothers were not
informed about their child´s diagnosis of CC by the
neonatologist. Only 6/22 (27.3%) patients that were not
counseled by the neonatologist were diagnosed before 1
yo. The mothers that were communicated of their
children´s disease by the neonatologist reached the
waiting list sooner, although the interval between the
Pediatrician/PP´s referral and PSU´s consultation was
not shortened (table 1).
No differences were detected between unilateral
and bilateral CC (mean and median ages at the waiting
lists 4.8 and 3 years for unilateral cases and 4.4 and 3.5
years for bilateral cases).
Mean age at the PSU´s consultation was 3.5
years (median 1.4 years). Mean delay time between
primary referral and specialized consultation was 1.6
years (median 0.6 years). The mean interval between

expert consultation and surgery/moment of the interview
was 1.1 year (median 1 year). Total waiting time between
primary consultation for cryptorchidism and surgery/
moment of the interview was 2.7 years (mean)(median 2
years). Waiting list time was justified by institutional
unavailability of operation rooms, except for 3 patients that
presented medical contraindications for surgery.
DISCUSSION:
Late orchidopexy is a recognized problem: many
authors attest 1/3 to ½ patients presenting cryptorchidism
are operated when > 3 years-old [5-9]. There is some
controversy about the reasons for that, as the problem
persists even in developed countries with universal health
systems where children receive excellent perinatal and
child care. The main justifications for late surgery vary
between AT, parents delay, late referral, unavailability of
surgery, public versus private health assistance and
anatomical factors (children with penile abnormalities and
nonpalpable testes frequently have earlier diagnosis and
are given priority) [10-11]. AT are the most frequent reason
for late surgery in advanced countries, attaining roughly
half of these cases [10] while inadequate referral or
unavailability of surgery may be the main motives in
underdeveloped nations [12].
Home delivery (and the absence of a Pediatric
Neonatal health examination) is not an explanation for late
diagnosis in our patients, as children in our cohort were
born in hospitals and had specialized Pediatric attention
as neonates.
AT are thought to represent approximately a
quarter of the cases of cryptorchidism, with a peak at 8-10
years of life [4, 10]. We can attest this specific diagnosis in
only 2 patients in our cohort (6.2%). However, AT is
probably not the explanation for late surgery in most other
cases, considering the age distribution of our population
(table 1, figure 2): AT typically attains school-aged boys
and is uncommon in toddlers and pre-schoolers. Other
authors also had diff icult ies about classifying
cryptorchidism in their cohorts: Hack et al were unable to
classify cryptic testes as acquired or congenital in 43.8%,
57.1% and 50% of 6, 9 and 13 year-old cohorts,
respectively [13].
Our study demonstrates that better practices
concerning the mother´s education about cryptorchidism
by neonatologists are needed and are an effective
strategy to ameliorate the early access of children with
cryptorchidism to treatment (table 1). A complete genital
examination, palpation of the testes and description of
their position is mandatory in any male neonate. The
mothers´ ignorance about the position of the testis in their
baby sons may be attr ibutable to inadequate
communication strategies between the health team and
the families or by misunderstanding of the team´s
orientations by the mothers. Both problems may be
corrected by specific protocols about cryptorchidism in
neonates and better education of Pediatricians and
families about the implications of the disease. This
research shows that a low proportion of children that were
not guided towards the correct treatment strategy of
cryptorchidism by the neonatologists were rescued and
diagnosed by Pediatricians/PP in the first year of life and
could be operated on during the ideal therapeutic window,
despite most children being sent to expert consultation as
soon as diagnosed by the Pediatrician/PP. There is proof
in the literature that education of neonatologists about
cryptic testes leads to a 2.5 y anticipation in the age of
surgery [14].
Boys presenting with cryptic testes at 6 months-
old should be immediately referred to a PSU for
treatment: early diagnosis and referral to a PSU has been
directly correlated to timely surgery [11-12]. Although age of
referral in our cohort varied a lot it was correct for many
children, the delay time between primary referral and

specialized consultation plus time in waiting lists for
surgery were long enough to determine late surgery. This
suggests that there are problems both in getting access to
expert consultations and in availability of orchidopexy.
We also showed frequent failure to diagnose
cryptorchidism in well-children routine examinations, as
about half of the patients look for consultations to evaluate
cryptorchidism as a family initiative or were incidentally
diagnosed by a PSU. Other authors also attest that
consultation about cryptic testes is frequently a family
initiative [9]. This may infer that either Pediatricians/PP
need more information about the disease or the routine
Pediatric physical examination lacks detailed genital
examination. This last problem is common in daily practice
even in countries with advanced health systems and a
high frequency of medical liability [15]. Another paper from
Brazil, which analyzed prospectively the cases of cryptic
testes in a Pediatric Surgery clinics from 1999-2000
confirms that the mother frequently responds for the initial
diagnosis of cryptorchism. In this paper most children
were diagnosed before 2 years of age but only 29% were
seen by a specialist and only 19% were operated on with
an ideal timing. In this paper the delay between evaluation
by the PSU and orchidopexy for most patients was ≤ 6
months [16].
Also, the knowledge that previously topic testes can
become cryptic (AT), which is relatively new information,
should be widely publicized [2].
No apparent prioritization was demonstrated for
bilateral cases. As fertility prognosis is worse in this
subgroup, we suggest that specific strategies for them
should be designed.
LIMITATIONS OF THIS RESEARCH:
1. Only 59.6% of the parents were available and accepted
to participate, although the ages of the excluded patients
were comparable to the studied cohort and this proportion
of exclusions has been commonly demonstrated in other
research projects using this methodology.
2. The two participant institutions are reference for
complex cases. This is the most probable explanation for
the high proportion of nonpalpable testes in our cohort, as
compared to the customary incidence in cryptorchidism
cohorts.
3. Our results should not be extrapolated to populations
with a large proportion of home deliveries that lack expert

4. As commented before, we cannot guarantee whether
the cases that were not detected as CC by the
neonatologists or classified as AT were cases of AT or CT.
To conclude, orchidopexies are delayed in Brazil because
of late diagnosis, long waiting times for consultation with
PSU and long waiting times for surgery (figure 3). The
situation may be addressed with better protocols and
education of Neonatologists, Pediatricians and PP about
the correct approach to infantile cryptic testicles, the need
of systematic genital examination in well-children
consultations and the possibility of acquired AT. Early
availability of PSU consultation and higher availability of
operating rooms for those patients are also essential.
Also, we would suggest prioritization of bilateral cases,
considering their worst fertility prognosis.

RESUMO
Lactente com 4 meses de vida apresentando
tumoração craniocervical à direita com crescimento
progressivo, cuja investigação radiológica sugeriu má
formação arteriovenosa. Submetido ao tratamento
c i r ú r g i c o c o m r e s s e c ç ã o t o t a l d a l e s ã o .
Anatomopatológico revelou tratar-se de hemangioma de
parótida direita. Hemangioma de glândulas salivares é
condição rara, porém mais frequente em crianças e
importante diagnóstico diferencial de massas cervicais
em lactentes.
ABSTRACT
Infant 4 months-old presenting right craniocervical
tumor with progressive growth, whose radiological
investigation suggested arteriovenous malformation. The
child underwent surgical treatment with total resection of
the lesion. The anatomopathological report revealed the
tumor to be a right parotid hemangioma. Hemangioma of
the salivary glands is a rare condition, but more frequent in
children and an important differential diagnosis of cervical
masses in infants.
INTRODUÇÃO
Entre os tumores de glândulas salivares, o
hemangioma corresponde a cerca de 90% nas crianças
menores de 1 ano e a parótida é a glândula mais
acometida, podendo apresentar de crescimento
progressivo com deformidade facial estética a disfunção
que ameaça a vida da criança.
RELATO DE CASO
Lactente masculino, 4 meses, com relato de
aparecimento de lesão facial a direita de aumento
progressivo há aproximadamente 2 meses. Ao exame
físico presença de lesão nodular endurecida no nível da
parótida direita, medindo cerca de 4x5x3cm, elástica, sem
sinais flogísticos (Figura 1).
A tomografia computadorizada de face evidenciou
aumento de volume de parótida direita com realce ao
contraste. A lesão se insinuava através do forame
estilomandibular, com múltiplos vasos nesta região. A
angiorressonância (RNM) sugeriu malformação
arteriovenosa com provável origem na artéria carótida
externa direita, volume aproximado de 92 cm3 (Figura 2).
Devido ao crescimento rápido e impossibilidade de
embolização optou-se pela ressecção cirúrgica. Foi feita
incisão à Blair com identificação de massa não
encapsulada, sem aderências aos planos de dissecção.
Identificado ramo vascular nutridor proveniente da
carótida comum e ligadura do mesmo com ressecção da
lesão preservando macroscopicamente o nervo facial
(Figura 3).
O paciente foi mantido com dreno de sucção no
pós operatório. No segundo dia de pós-operatório
apresentou sangramento importante, sendo necessária
reabordagem cirúrgica de urgência, quando foi
identificada uma laceração em carótida externa direita,
sendo a mesma rafiada. Na evolução apresentou
paralisia facial à direita e desvio da rima à esquerda com
melhora gradual completa após fisioterapia, recebendo
a l t a n o 1 2 º d i a p ó s - o p e r a t ó r i o . O e x a m e
anatomopatológico diagnosticou hemangioma da
infância em parótida. Segue em acompanhamento
ambulatorial sem intercorrências após 9 meses de
seguimento (Figura 4).
DISCUSSÃO
Os hemangiomas são os tumores benignos mais
comuns na infância, acometem 4 a 5% das crianças, e
são diagnosticados em 1 a 2,6% dos neonatos. A
incidência é de 4 a 12% das crianças no primeiro ano de
vida, com idade média de apresentação aos 4 meses, 2 a
4 vezes mais em meninas do que meninos 1,2,3. Origem
caucasiana, prematuridade e baixo peso são fatores de
risco para apresentação de hemangioma 4,5. Até 80%
deles se apresentam na cabeça e no pescoço, sendo o
hemangioma de glandula parótida uma condição rara,
correspondendo a 1% dos tumores das glandulas
parótidas 5,6. Entretanto, o hemangioma corresponde a
cerca de 90% dos tumores de glândulas salivares nas

crianças menores de 1 ano. A parótida é a glândula
acometida em 80-90% por hemangiomas 7,8,9,14.
Apresentam-se, em geral, como massas unilaterais, e
podem acometer a pele sobrejacente8.
A avaliação complementar inicial é realizada por
ultrassonografia com doppler, método eficaz não
invasivo e sem radiação ionizante. RNM é raramente
necessária para confirmação, embora seja padrão ouro
para o diagnóstico 7,10. Os diagnósticos diferenciais são
má formação arter iovenosa, h igroma císt ico,
rabdomiossarcoma, f ibrossarcoma congêni to,
miofibromatose e sialoblastoma 9.
Após curta fase de crescimento em geral entre os
4 a 12 meses de vida, chamada fase proliferativa, os
hemangiomas tendem a uma fase de regressão
espontânea nos primeiros anos de vida em 85-90% dos
casos, que se conclui em geral na primeira década,
sendo o manejo conservador inicial preconizado em
geral 2. Entretanto os pais devem ser bem orientados
porque a involução do hemangioma pode levar muitos
anos 11.
O tratamento conservador pode ser expectante
ou farmacoterapia com corticoides orais ou injetáveis,
interferon, vincristina, bleomicina, e mais recentemente
propanolol ou terapia com laser 2,3,11. O propanolol tem
se mostrado eficaz no tratamento conservador, com
baixo custo, boa eficácia e poucos efeitos colaterais
associados 1,2. Os hemangiomas de parótida segundo
alguns estudos parecem apresentar menor resposta a
farmacoterapia que aqueles em outras áreas do corpo,
sobretudo os maiores 13,14. Contudo respondem na
maioria dos casos, com estudos mostrando que 83%
responderam com corticoterapia sistemica, 95% com
interferon 14.
Porém, em 5-20% ocorre má resposta ao
tratamento conservador ou mesmo complicações
precoces, que demandam tratamento cirúrgico.
Crescimento exuberante causando assimetria e
deformidade da face, ulceração, sangramento e
obstrução do canal auditivo são as mais frequentes. Em
alguns casos grandes massas tumorais podem causar
obstrução das vias aéreas ou sobrecarga cardíaca por
shunt, tornando-se reais ameaças à vida 2,13.15.
Os tumores não são encapsulados, infiltram

CONCLUSÃO
A ressecção cirúrgica pode ser indicada em casos
d e f a l h a d o t r a t a m e n t o m e d i c a m e n t o s o ,
comprometimento das vias aéreas, ulceração, além de
problemas estéticos e psicológicos. A cirurgia está
associada a benefício estético e baixa morbidade, no
entanto é necessária cautela na indicação de uma
parotidectomia tendo em vista o risco de dano ao nervo
facial e a dependência de um cirurgião experiente.

tecidos adjacentes e sua ressecção é delicada, com risco
real de lesão a estruturas importantes como nervo facial e
vasos sanguíneos de alto fluxo associados ao sistema
jugular/carotídeo. A ressecção cirúrgica do hemangioma
de parótida é melhor indicada na fase de regressão em
casos de falha do tratamento conservador, complicações,
para tratamento de problemas estéticos e psicológicos,
por volta dos 3 a 5 anos. Em sua maior parte os
procedimentos cirúrgicos são realizados com fins
estéticos para remoção de mácula ou excesso de pele e
revisão do canal auditivo 13.
Em alguns estudos, 66% dos pacientes
necessitaram de cirurgia, mas na fase de regressão 14. A
ressecção cirúrgica na fase proliferativa deve ser
realizada somente nos casos com ameaça de vida, como
no comprometimento das vias aéreas 2,14.
Este caso ilustra como é fundamental considerar
os diagnósticos diferenciais tanto no tratamento clínico-
farmacológico quanto no cirúrgico e na escolha entre os
mesmos. Exemplifica ainda que até exames de alta
sensibilidade e especificidade como a tomografia
computador izada podem suger i r d iagnóst icos
equivocados, o que neste caso determinou a antecipação
de uma provável ressecção cirúrgica ainda na fase
proliferativa.
A parotidectomia total ou parcial deve ser
executada por um cirurgião experiente com domínio da
anatomia da região tendo em vista o risco de

os beneficiários em curtíssimo prazo. Você sabia disso?
Além disso, é possível blindar seu patrimônio contra
ações cíveis ou trabalhistas. Não há blindagem para
processos criminais, claro. E o investidor é avaliado antes
sobre a origem dos recursos investidos, mas se estiver tudo
ok, o investimento pode ser feito e protegido.
No Brasil há o FGC (Fundo Garantidor de Crédito)
quando uma instituição tem problemas financeiros e não
cumpre com as regras acordadas com o investidor, porém a
garantia pode ser bem mais abrangente lá fora.
POR QUE PENSAR EM INVESTIR NO EXTERIOR?
É legal investir no exterior desde que devidamente
informado na declaração anual de imposto de renda e
notificado ao Banco Central caso o valor seja superior a US$
100.000,00. As remessas precisam ser feitas dentro das
regras e a origem do dinheiro tem que estar dentro da
legalidade.
Por que é permitido a um brasileiro investir no
exterior? Pelo mesmo motivo que o Brasil quer atrair um
estrangeiro a investir aqui. Reciprocidade internacional!
Há opções de investimentos internacionais
compatíveis com os diversos tamanhos de bolsos dos
investidores.
Há jurisdições que permitem mudanças de fundos
sem custos e não cobram impostos regionais, o que dá uma
boa vantagem durante o período de investimento. Os
impostos precisarão ser recolhidos no Brasil, no momento do
resgate, não importando se o dinheiro veio para uma conta
nacional ou não.
No exterior e no Brasil, existem instituições sólidas e
competentes, assim como o inverso. O importante é escolher
corretamente.
Investir no nosso país é fundamental. Investir
somente no nosso país, porém, limita o conceito de
diversificação em termos geográficos. No Japão, por
exemplo, cuja economia é bem mais estável e está na casa
dos 8% em relação ao mundo, é comum a diversificação
geográfica.
Crises mundiais afetam a todos. Aí, não tem jeito! Se
a crise for regional, por outro lado, há como escolher outros
locais e nichos onde a economia continua sólida.
Se você tem objetivos familiares relacionados ao
exterior, no médio ou longo prazo, faz mais sentido ir juntando
em moeda nacional ou na moeda de destino?
Quando você pensa em uma viagem de turismo que
limites você se impõe? Você explora opções pelo mundo ou
se restringe ao Brasil? Quando se trata de investimentos por
que ser diferente? Já pensou sobre isso antes?

A Revista da CIPERJ aceita artigos em português, inglês ou espanhol que sigam as Normas para Manuscritos Submetidos às Revistas Biomédicas, elaborados e publicadas pelo International Committe of Medical Journal Editors (ICMJE www.icmje.org). De forma geral: Editorial: Geralmente solicitado pelo editor ao autor. Artigo Original: É o relato completo de uma investigação clínica ou experimental e seus resultados. Constituído de Resumo/Abstract, Introdução, Método, Resultados, Discussão, e Referências (máximo de 30). O título deve ser redigido no mesmo idioma do manuscrito e não deve conter abreviaturas. Deve ser acompanhado do(s) nome(s) completo(s) do(s) autor(es) seguido do(s) nome(s) da(s) instituição(ões) onde o trabalho foi realizado. Se for multicêntrico, informar em números arábicos a procedência de cada um dos autores em relação às instituições referidas. Os autores deverão enviar junto ao seu nome o título que melhor represente sua atividade acadêmica. O resumo deve ter no máximo 250 palavras ser estruturado com objetivo, método, resultados, conclusões e até 5 descritores na forma referida pelo DeCS (http://decs.bvs.br). O abstract deve ser estruturado da mesma forma que o Resumo e seguido pelas keywords conforme o sistema MESH (PUBMED). Devem conter no máximo 2.420 palavras e 45 referências. Artigo de Revisão: Versando sobre temas de interesse para a Cirurgia Pediátrica, incluindo uma análise crítica. Não serão aceitas simples descrições cronológicas da literatura. Devem ser descritos na introdução os motivos que levaram à redação do artigo e os critérios de busca, seguidos de texto ordenado em títulos e subtítulos de acordo com complexidade do assunto. Resumo e abstract não são estruturados. Devem conter no máximo 2.420 palavras e 45 referências. Nota Prévia: Observação clínica original ou descrição de inovações técnicas apresentadas de maneira concisa, não excedendo a 500 palavras, cinco referências, duas ilustrações e abstract não estruturado. Até 3 autores. Relato de Caso: Descrição de casos clínicos de interesse pela raridade, pela apresentação ou formas inovadoras de diagnóstico ou tratamento. Máximo de 1.420 palavras. Resumo e abstract não estruturados, até 15 referências e duas ilustrações. Máximo de 5 autores. Cartas ao Editor: Comentários científicos ou controvérsias com relação aos artigos publicados na Revista da CIPERJ. Serão aceitos ou não para publicação, a critério do editor, sem passar pelo processo de peer-review.Texto: Os manuscritos devem ser inéditos e enviados na forma digital (Word), espaço duplo e letra arial tamanho 12. As imagens deverão ser encaminhadas separadas no formato JPG, GIF ou TIF, e as legendas serão apresentadas em página separada no manuscrito As abreviaturas devem ser limitadas a termos mencionados repetitivamente, e definidas a partir da sua primeira utilização. Referências: Em ordem de citação, numeradas consecutivamente e apresentadas conforme as normas de Vancouver (Normas para Manuscritos Submetidos às Revistas Biomédicas - ICMJE www.icmje.org ). Não serão aceitas como referências comunicações pessoais. Citações de livros e teses devem ser evitadas. Agradecimentos: Devem ser feitos às pessoas que contribuíram de forma importante para a realização da pesquisa ou escritura do manuscrito.TABELAS E FIGURAS Numeradas no corpo do texto com algarismos arábicos, encabeçadas por suas legendas, explicações dos símbolos no rodapé. A montagem das tabelas deve seguir as normas supracitadas de Vancouver. São consideradas figuras fotografias, gráficos e desenhos. Todas as figuras devem ser referidas no texto, numeradas consecutivamente por algarismos arábicos e acompanhadas de legendas descritivas.
