Cisto Ósseo Simples • Faixa Etária:
o Mais comum em homens que em mulheres, é observado durante as duas primeiras décadas de vida.
o Pcts mais idosos, há aumento da incidência em locais atípicos, como calcâneo, tálus e ílio.

o Lesão radiotransparente, central, bem circunscrita com margens escleróticas.
o Não há reação periosteal (aspecto que distingue do cisto ósseo aneurismático).

• Causa: traumatismo ou alterações na hemodinâmica local.
• Pode surgir como lesão primária ou como alterações císticas de várias lesões benignas e malignas

o Observados predominantemente em crianças (90% das lesões ocorrem com menos de 20 anos de idade).
• Localização: o Preferencialmente na metáfise

Secondary aneurysmal bone cyst. A 14-year-old boy had a painless swelling on the dorsum of the left hand. Dorsovolar film of the hand shows an expansive lesion in the distal segment of the third metacarpal. The lesion exhibits a well-organized periosteal reaction; the articular end of the bone is spared. • Diagnóstico Diferencial: o Cisto ósseo simples:
• COA é uma lesão expansiva, excêntrica, invariavelmente associado a algum grau de reação periosteal,
• COS é uma lesão central, com pouca ou nenhuma expansão e sem reação periosteal.

Cisto Ósseo Aneurismático • Diagnóstico Diferencial:
o Fibroma Condromixóide: • Pode ser indistinguível do COA, pois ambas as
lesões são excêntricas, expansivas e geralmente afetam a metáfise, exibindo uma margem de esclerose reativa e reação periosteal sólida.
o TGC: • Pode mimetizar o COA, embora geralmente não
esteja associado a uma reação periosteal e raramente exibe uma zona de esclerose reativa.

Tumor de Céls Gigantes (TGC)
• É uma lesão agressiva caracterizada por tecido ricamente vascularizado.
• Localização: o 60% ocorrem nos ossos longos,
preferencialmente tíbia proximal, fêmur distal, rádio distal e úmero proximal
o Quase todas estão localizadas na superfície articular do osso.

Tumor de Céls Gigantes (TGC)
• Faixa Etária: o Observados quase que exclusivamente após a
maturidade óssea, quando a placa de crescimento está obliterada (entre 20-40 anos).
o Predomínio no sexo feminino (2:1).
• Aspecto Radiológico: o Lesão radiotransparente puramente osteolítica que
não possui margens escleróticas e geralmente requer reação periosteal.

Giant cell tumor. Dorsovolar radiograph of the left wrist of a 56-year-old woman shows a giant cell tumor of the distal radius that has destroyed the cortex and that extends into the soft tissues. Despite this aggressive radiographic presentation, on histopathologic examination the tumor had a typically benign appearance, without malignant features. After wide resection, a 5-year follow-up showed no evidence of recurrence or of distant metastases.

Hemangioma • Locais mais comuns:
o Coluna vertebral, particularmente segmento torácico
o Crânio • Na coluna, a lesão
tipicamente envolve o corpo vertebral, embora possa estender-se até o pedículo ou lâmina

Hemangioma • Faixa Etária:
o Sua incidência com aumento da idade. o Mulheres 2:1 Homens
• Aparência Radiológica: o Focos líticos multiloculados ou estriações verticais
grosseiras, denominado em “favo de mel” ou em “tecido canelado” quando acomete a coluna e “em roda denteada” quando acomete o crânio.
o Quando observado na coluna, este padrão é considerado praticamente patognomônico.

• Os pcts com este distúrbio apresentam lesões líticas solitárias ou múltiplas.
• É denominada marrom devido ao seu conteúdo fibroso com sangue em decomposição.
• Localização: mais comumente nos ossos tubulares longos e curtos

Tumor Marrom do Hiperparatireoidismo
• Diagnóstico: Lesão + anormalidades associadas, que incluem: o Osteopenia o Reabsorção óssea subperiosteal, melhor observada
na face radial das falanges proximal e média do 2 e 3 dedos
o Aspecto granular de “sal e pimenta” da abóbada craniana
o Reabsorção das extremidades acromiais das clavículas
o Calcificação dos tecidos moles.

(Granuloma Eosinofílico) • Pertence ao grupo de distúrbios conhecidos
como reticuloendotelioses (ou histiocitose X), um grupo que inclui 2 outros distúrbios: o Dça de Hand-Schüller-Christian (xantomatose) o Dça de Letterer-Siwe (reticulose não lipídica).
• A histiocitose das céls de Langerhans agora é considerada um distúrbio da regulação imune, e não um processo neoplásico.
• Exibe um amplo de espectro de anormalidades clínicas e radiológicas.
