Patients FA :

Management of bleeding in anticoagulated patients
NOAC
Minor
Moderate - Severe
Severe orlife-threatening
Patient with active bleeding
Compress bleeding sites mechanically
Assess haemodynamic status, blood pressure, basic coagulation parameters, blood count, and kidney function
Obtain anticoagulation history (last NOAC / VKA dose)
VKA
Delay VKA until INR <2 Delay NOAC for 1 dose or 1 day
Add symptomatic treatment:
Fluid replacementBlood transfusionTreat bleeding cause(e.g. gastroscopy)
Consider to addVitamin K (1–10 mg) i.v.
Add symptomatic treatment:
Fluid replacementBlood transfusionTreat bleeding cause(e.g. gastroscopy)
Consider to add oral charcoalif recently ingested NOAC
Consider PCC and FFP
Consider replacement of platelets where appropriate
Consider specific antidote, orPCC if no antidote available
Consider replacement of platelets where appropriate

INR 1,2

Normal haemostasis during surgery was reported in 92% of the patients that required surgery or invasive procedures
Normal intraoperative hemostasis was reported in 33/39 patients
Mildly or moderately abnormal hemostasis was reported in 2 patients and 1 patient, respectively.
RE-VERSE AD™: Mortalité dans les Groupes A et B
18 deaths overall +++
Mortality within 96 hours of
study enrolment appeared
to be related to the original
reason for emergency
admission to the hospital
All later events appeared to
be related to co-morbidities
Pollack et al. N Engl J Med 2015
RE-VERSE AD™: accidents thrombotiques
Thrombotic events reported in only five patients over a period of 90 days of follow up• 1 early event (DVT + PE) 2 days after idarucizumab
administration
• 4 events 7–26 days after idarucizumab administration
• DVT: 7 days
• DVT + PE + left atrial thrombus: 9 days
• MI: 13 days
• Ischaemic stroke: 26 days
• None of these 5 patients were receiving any antithrombotic therapy when the events occurred
These events reflect the underlying thromboembolic potential of these patients when not on anticoagulation therapy
