I declare that I have decided to join the 2019 Nutrilite™ Weight Management Competition of my accord and that I am obliged to follow the following competition rules:1 I am willing to bear all responsibility for any injuries or other consequences, including death, incurred

during the competition. Amway Hong Kong does not hold any responsibility in relation to any relation to any of the above cases.

2 I declare that I am healthy enough to take part in the competition.3 I understand the physical requirements of the competition before joining.4 I agree that the organizer has the right to use my portrait, name, recording, video footage, interview

and personal information for promotional use.

謹此聲明本人自願參加2019紐崔萊™體重管理比賽，本人願意遵守是次比賽所訂的規則，包括：

1 我願意自行承擔所有責任，本人一旦因在比賽中健康受到任何影響、受傷或死亡，安利(香港)

毋須負上任何責任。

2 本人聲明身體健康及有能力參加此活動。

3 本人明白此活動的健康要求，方報名參加活動。

4 本人同意大會在毋須經本人審查而可使用本人之肖像、姓名、錄音、錄像、訪問及個人資料，作為

大會活動籌辦及推廣用途。

1 體重過輕者(BMI<18.5)、未滿18歲及65歲以上人士、孕婦及患有特殊疾病者將不被接納參加是次比賽。

2 參加者必須自行以雙腳站立於脂肪磅上進行量度。3 減重量度單位將以小數點後一位計算，減重後BMI不能少於18 .5，否則將被

取消參賽資格。4 第一階段之比賽必須以3至4人為報名單位，隊名及隊員資料一經提交， 不得更改。

5 同組之每位參賽者必須於相同日期完成報名及結果量度程序。結果量度日期不得更改。

6 量度結果時必須脫去外套及卸下身上所有重物，在有需要時，安利職員有權檢查參賽者有否違規。

7 每次量度只可上磅一次，同一天不得再次量度。8 若參賽隊伍由4人組成，團隊成績將只計算隊伍中成績最好之3位成員。9 每名參賽者須於每階段各購買至少一套2019紐崔萊™體重管理套裝，以參加比賽。如欲參加第二階段之比賽，須於第一階段結果量度日報名，並於報名當天提供有效購物收據，逾期遞交恕不受理。

10 參賽者必須提供有效購物收據以獲取參賽資格，有效收據包括：於2019年5月3日或之後發出的直接購貨訂單正本、臨時送貨收據正本或已付款之網上訂單。如果發現收據經過偽造或被重用，該參賽者及其隊員將直接被取消參賽資格，而不作另行通知。

11 6公斤團隊纖體獎得獎隊伍不可同時獲得3公斤團隊纖體獎。12 個人輕盈體態獎共設六個獎項，若多於一人的減重百分比(計算至小數點後2個位)結果相同，將以實際減重磅數較多者為勝出。

13 個人最佳「脂肪率」表現獎共設六個獎項，若多於一人的減去脂肪百分比(計算至小數點後2個位)結果相同，將以實際減重磅數較多者為勝出。

14 個人輕盈體態獎及個人最佳「脂肪率」表現獎優勝者必須符合最少減去3公斤之體重要求。

15 3公斤或6公斤團隊纖體獎得獎者可同時領取個人輕盈體態獎、個人最佳「脂肪率」表現獎。

16 健康減重是以飲食控制、適量運動與良好習慣達成瘦身目的，參賽者嚴禁使用不當方法或藉藥物減重達至瘦身效果。若經發現透過不當方法達至減重效果，該參賽者將直接被取消參賽資格，而不作另行通知。

17 美國安利(香港)日用品有限公司保留是次比賽之最終決定權。

1 Participants with a BMI of less than 18.5, those aged below 18 and above 65, with certain illnesses and pregnant women are not eligible to join the competition.

2 Participants should stand on the body fat analyzer by himself without any assistance during result measurement.

3 Measurements will be counted to one decimal place. Participants with a BMI of less than

4 Applicants must form a group of three or four to enroll in the 1st competition stage. No change of the team name and team members’ information will be accepted once the enrollment is submitted.

5 Participating group members should enroll and have the result measurement conducted on the same date. No alteration of result measurement date will be accepted.

6 Participants must take off coats and remove all heavy items for the result measurement. Amway staff is entitled to examine any violation of the relevant rules if necessary.

7 Only one measurement will be held each time; repeated measurement is not allowed in the same day.

8 If a group has four participants, only the results of the three best-performing participants will be calculated.

9 Each participant needs to buy at least one 2019 Nutrilite™ weight management promotion package for each competition stage. To enroll in the 2nd competition stage, a valid receipt must be presented on the 1st stage result measurement date. Receipts submitted afterwards will not be accepted.

10 Participants must present valid receipts for enrollment. Valid receipts include the original receipts of on-site paid orders, temporary delivery orders or paid online orders issued on or

competition without prior notice once a fake or reused receipt is found.11 Winners of the 6kg Group Slimming Award are not eligible for the 3kg Group Slimming

Award.12 For the Individual Slimming Award, if more than one participant has the same weight loss

percentage (rounded off to the second decimal place), the participant with the greater body weight loss shall be declared the winner.

13 For the Individual Best “Body Fat Percentage” Award, if more than one participant has the same body fat loss percentage (rounded off to the second decimal place), the participant with the greater body weight loss shall be declared the winner.

14 Winners of the Individual Slimming Award and Individual Best “Body Fat Percentage” Award should lose at least 3kg.

15 Winners of the 3kg or 6kg Group Slimming Award are also eligible for the Individual Slimming Award, Individual Best “Body Fat Percentage” Award.

16 Healthy weight management equates to a balanced diet, adequate exercise and good r notice if found to have lost weight

by using slimming drugs or through other inappropriate methods.17
